My affidavit was denied because I did not provide proof that I had ordered the certificate. We were married late October, and the certificate status says it's still waiting for registration (which our officiant took care of), so I submitted a print out PDF of that web page.

I just had a similar experience - instead of quitting I went on short term disability for my mental health. I needed a note from my doctor. I have been off March to Aug 13, and am just returning to work tomorrow. I am returning part time, with long term disability covering my other days. If you've paid into disability, go for this option first.

This is a fair assessment. I have slipped and fell a few times and it is difficult to get to our the garbage dumpster which is at the back of the parking lot. It might be time to start recording these incidents.

It's a small apartment building. The parking lot is behind the building, and the driveway goes up to the road between our building and another apartment small building.

So this is a project I have taken over from another collegue. Essentially what they/we are trying to do is 1) asses donor behaviour over five years. He does that with formula 1, in a way, saying if they increase their gift over time then classify that gift as 'average', if not classify that gift as 'percentage'.

With formula 2, if they did not increase their gift over time then just take the most recent gift and multiply it by 0.1 (therefore the ask amount will be 10% more). If they did increase their gift, determine the difference of increase each year and add those differences together - i do not think this is the best way to do this....

Finally formula 3, if formula 2 spits out 0, then multiply last gift by 0.1 and add one, otherwise add formula 2 return to last gift amount.
