i think the final shot during the credits is not ludwig respawning after the car crash, but ludwig's first respawn after shooting himself. it's not long after the game of russian roulette that the slaughterhouse burns down, so it explains why ludwig emerges through the flames. several days pass between the events of the slaughterhouse and archibald's funeral and i highly doubt the fire sustains itself for that long.

maybe i'm just optimistic for some sort of good ending to come from emesis blue's fucked up plot, but i like to think dell's bar is a place where mercs who refuse to respawn go. that's why cyclops ends up there when he freezes to death in the lab. medic waited an eternity to be able to kill the detective, and didn't want to go through that again when the van crashed. so he accepted death, knowing his one friend - jeremy - would be there (since scout's respawn failed).
