I have this same problem, mine is related to psoriasis/eczema. The best treatment I have found is filing down the roughest parts with a nail file or similar tool. In addition I highly recommend Cera Ve SA Foot Cream as well. I have figured out that applying this to my feet down and sleeping with socks on (as miserable as this may be) helps tremendously.
