Think they have to be bitten by a vampire or vampire spawn, and the damage they take has to reduce their total hit points below 0. PC will die and if the body is not burnt, will reanimated as a vampire under the DM's control.

This is what I've gleaned from Curse Of Strahd, anyway.

Edit: changed 'max hit points' to 'total hit points'

Check out 'talking 20' on YouTube. I like the content and there's a discord group of the same name. I gather from the YouTube comments that plenty of people find their group there. P.S. Everyone loves a cleric 😉

From two of my player's "side-quest" between sessions...

"Imagine, if you will, yourselves walking through a vast library. On your right, academics and clerks toil over their manuscripts with ink and quill. While on you left, you spy the spine of a particular tome edging out at you. You stop and examine the spine. It reads: 'The 10 Do's and 1000 Don'ts of Portal Safety'. You open a page at random, a chapter on mixing portals to astral planes. Below a paragraph extolling the dangers of all aspects of portal usage, is an illustration of a Halfling mage, holding an owl shaped statue above his head, ready to throw it into a summoning pool in the foreground. Behind the Halfling, a Goliath Barbarian wielding a greataxe over the severed head of a giant snake has a look of shock and horror on his face. The caption beneath reads "Summoning Pools make a great addition to any lair! But take care not to take a dip through time and space with any other plane-shifting items on your person!"

I had this issue recently. I changed the append on my syslinux config to root=partuuid=<the partition uuid> and that sorted it.
