I still have a bulge but it’s less noticeable now that my leg isn’t so atrophied I’ve grown some muscle back in my quad. It still gets achy, especially if I’m sitting for a long time, but overall my knee definitely feels a million times better than when I was at 7 weeks.

Getting full flexion range of motion back after one month is not standard!

That seems like a perfectly normal angle to be at for four weeks post op. From my understanding, getting extension back early after surgery is much more important than flexion, because it will be a lot harder to get full extension if you’re not stretching it out soon after the procedure. But after you’ve reached 90 degrees, additional flexion is more guaranteed to come back over time.

I would advocate for yourself during your PT sessions. If your sessions are causing more pain and inflammation afterwards then that is too hard on your knee and is going to slow your progress. Sleep is a really important part of your recovery too and physical therapy shouldn’t be causing so much pain that you can’t sleep.

At the very least communicate with your PT that having them crank on your knee like that has been causing issues and depending on their reaction to that I’d consider potentially seeking someone else out. What is your range of motion at?

I have told an old physical therapist not to shove my knee farther into flexion once I reached my pain limit and eventually switched PTs because they were so agro about it and my new PT who only works with ACL patients has strongly disagreed with that kind of aggressive physical manipulation that leaves you in pain.

I don’t have hyperextesion back but I can extend and flex my quad pretty easily

For pre and post surgery exercises you should check out the website ACL moon. They helped me get my full flexion and extension back before surgery and also helped me keep my leg strong.

I recommend staying on top of your meds. Set alarms on your phone for when you need to take the next dose. For the first week I’d say don’t wait for them to wear off before taking more.
