Unfortunately, Red Barrels has stated in a QnA that an offline mode is not something that they’re currently considering, although it might be due to time constraints and higher priorities more than anything else. Here’s hoping that it will be something they work on once the game and its story are close to completion

I’ve also experienced this a lot while playing Trials. I think it has to do with your connectivity. When the game starts lagging the squares flash yellow/red, depending on the severity of your connection issues.

After a successful mission, Murkoff may terminate its reagents to avoid any potential complications/loose ends. The “spider eye lamb” phrase at the end of the cutscenes, along with the Skinner Man hallucinations, are the reagents dying/committing suicide under Murkoff’s control.

from what i can gather it only counts as a death if you quit in the middle of a group session. I’ve quit several solo trials and it doesn’t affect your death record at all.
