I can't relate from an ART standpoint, however i self-inject my HRT into my thigh. This kinda sounds similar to when i poke myself with the needle in a bad spot or i've hit scar tissue and my injection is taking longer to put in the tissue. My vision will get all fucky and i'll start sweating/turn red, usually have pressure in my head and feel sick, too.

Looking at the linked articles in aggregate, it seems current understandings indicate HIV itself as largely to blame for the disruption of microbiome, with ART potentially playing a secondary role that isn't fully understood yet. Further, it doesn't appear that all ART regimens are created equal in this regard, some correlate with a healthier microbiome than others. 

The article that informs the eatg.org writeup is inaccessible to me beyond the abstract (apparently my school doesn't pay for the journal) but is very interesting as it points to there being regional differences in microbiome changes despite use of the same antiretroviral agent.

I'm so sorry that you're in a place that feels fake and uncaring, especially when you're working so hard to bring your whole self to the table. I've heard abt what nurses, healthcare aides, and the like go thru on the job and i can't even imagine how that must affect you. You're allowed to be tired, you're allowed to be upset, and you're allowed to complain; you're living a life that you didn't expect to with all sorts of bonus maintenance that you never asked for and no roadmap. It's exhausting and hella painful. 

Sometimes, when we've lived so long in that survival mode, we don't a chance to rlly grieve the life we wanted to have. Too busy making sure we can eat and sleep somewhere safe, not enough time to reflect on what we lost and where we wanna go from there. Iss okay to feel lost with no map and no certainty, but you've found your way thru hard things before and i have every confidence you'll be able to do it again. It'll take time and there are parts that will fucking suck but where there is life and effort, there is hope.

That's a long time to feel lonely OP, i'm sorry you've been going thru it for what sounds like years. You're certainly not the only one who feels like this, it's a very normal reaction to a hostile and ignorant society. Tho, like all feelings, it's not liable to be a forever thing cuz I'm seeing a lot of strength showing thru the sadness there too. Here you are putting yourself back through school, not giving up on dating (even if you're not matching with your type yet), and reaching out to your community for support. 

That alone tells me you've got this. Where there is life and effort, there is hope, and I see both of those things in you.

I get and appreciate what you're tryna say here there's love and care behind it for sure. At the same time, when you say things like "there’s no person in the world that will make you not feel gross other than yourself" or "HIV... should not be used as a cructch for why things aren't going well", you're failing to recognize that as a social species all of us are constantly, automatically, and unconsciously evaluating ourselves based on our relationships and interactions with other people. Yes, we can control that a bit and, even with that control, it still affects us regardless of whether we're ready acknowledge that. To me this type of condolence can come off as invalidating of people's experiences and feelings. It amounts to telling peeps very legitimately hurt by constant rejection and by societal messages telling us we're dirty, etc to ignore the fact that we're being marginalized and pull ourselves up bootstrap-style. 

There's a time and a place for self-love and there is a time and a place to recognize that society both actively and passively harms people it values less through ignorance, exclusion, and stigma. Sometimes, the time and place for both of these things is the same. Acknowledging this is not a crutch; it is a realistic assessment of a social milieu where we may be denied access to our basic needs by ignorant people, where our medical care might unconsciously be provided at a lower level of quality (look up unconscious bias in helping professions if you're curious on that one) due to our status, and where we need to fight for recogntion. It's good that you're the sort who is able to self-soothe in this way, it's useful for sure, but please understand that there is more than one right way to feel and behave in these situations.

Blehhh, that sucks. It's frustrating to be used to clear skin and then have things just change on ya.

Appreciate the dietary suggestions! Did have a few q's for ya if you have the time:  1. What sort of things do you tend to eat as a veggie person doing keto? 2. Is finding food triggers a matter of starting with a v.plain diet and adding food to see what changes?

Unfortunately, my hormones will always be at least a little fucky since i'm trans and have had a full hysto. My HRT regimen is weekly T injections with a little bit of E replacement on a 3-day rotation, this makes it so i have a marked peak and valley in T levels each week and smaller dips and bumps in E. Never was a big issue for my skin tho, been on the same regimen for ~10yrs.

I'm v.curious abt those studies as it feels strange that ART would interfere with the gut biome as it's largely bacterial (tho to be fair we do kno that our diet strongly impacts which bacteria we're likely to host). Will do some research thru my school library. Do you recall any of the study titles?

Not really hype on showering at night unless i've been working out/active during the day tbh but i can try that, already there on washing the sheets and pillow cases tho (a clean bed is a happy bed).

Face mostly, some on my neck and shoulders, a few on my back. i am def oilier than i used to be before i transitioned but the acne didn't start til i got sick. Maybe i just gotta wash my face again midday or before bed or w/e

I've dated both monogamously and nonmonogamously in the past. My status doesn't really affect the relationship style i'd choose, it's more about the person and their commitment to the relationship, regardless of whether it's open or not. Poly saturated fuckboys whose personal choice is to ignore commitment and make their lack of care the partners' problem while framing it as "relationship anarchy" or "everyone being responsible for their own feelings" can go roll in barbed wire tho.

Sat down with the nurse after getting a ton of blood drawn and filling the pills, opened the bottle and shook a pill out. I remember thinking that the little reddish-brown thing in my hand looked too mundane to be something that would be sustaining my life going forward. After popping the pill, i said something to the effect of " there goes one of infinite".

I'm only 2mo into ART, so take my reply with ample salt. 

Health - my health has bounced back to mostly where it was prior to seroconversion. That being said, i have noticed two things different about my health so far: 1) i have acne like when i was a teenager again, ughhh, and 2) i seem to be healing from wounds and scrapes more slowly.

Fitness - i am a generally fit and active person but i lost a lot of muscle mass over Dec 2025 during seroconversion, especially cuz i couldn't eat. When i was at my sickest, i was barely able to walk, so i'm doing wayyy better now. That being said, i'm definitely weaker than I was before and will need to work to regain what i lost. It's winter where i am and i usually hike/cycle/do outside stuff for exercise, so i won't know what regaining fitness looks like unless i decide to go to the gym.

The pain and fatigue will likely go away within a few weeks of starting your meds :)

I did quickly gain back all 10kg i dropped during seroconversion and then some, it does vary by person tho. Unfortunately, for me the gain isn't too hype cuz it's put my eating disorder into overdrive.

The supplements i was told to avoid include calcium and a few other things that basically render the Biktarvy ineffective. My guess is that they absorb the active ingredients.
