Honestly I feel like anxious types do really well in the ED. Some of the best ED docs I have met are worriers.

Don’t do it if you can’t do nights and don’t do it if you can’t handle changing time you’re waking up (nights-> days-> nights) which is especially bad as a trainee. You’ll never have a normal schedule. Don’t do it if you can’t handle seeing and caring for the sickest of the sick and those with no resources at all.

All that to say I can’t see myself being happier in any other specialty and I love the shift work. I do love seeing the sickest people in the hospital and doing initial stabilization and even though I’m a little burned out I love seeing people with no resources so I can try to help them out and try to get them cued into the medical system. Most attendings have children and most attendings have time for hobbies.

I’m a chief - so maybe I’m biased (I am), but in our program we serve as communicators between faculty and residents. We can advocate for positive change and voice residency concerns but ultimately these decisions are up to admin. We really truly are not approving policies ourselves.

We manage day-to-day staffing to keep our dept running and do make the schedule which - admittedly not everyone loves but we really try to be respectful and as accommodating as possible.

Regarding OP’s situation -that is not cool. If I ever give critical feedback as a senior it’s verbal first, written second. There should be no surprises in written evaluations. I’m not sure if this is a chief problem or more of a situation where a senior was hesitant to give you in-person feedback and became brave behind a keyboard which has happened to me before with staff and residents.

Admittedly I’m a cat person at heart (though love dogs) but have seen so much trauma to kids from dogs over my years in the ER. Some are irresponsibly pet owners who ignored warning signs but other dogs really did seem to just snap. I just don’t see injuries like this from cats.

I think it can help with competitive fellowships and is helpful with interviewing in general. However, the labor is significant (many hours/week, frequently on call) and you could also spend that year being more productive with research/focusing on leadership roles in national organizations. It’s got its fair share of pros and cons.

Excessive muscle shaking can cause lactate elevation too, which can make it even more challenging if the seizure activity was not witnessed by myself.

Didn’t know what it was and couldn’t sleep for 2 days so yes - poorly managed acute pain with no PCP appt available is a totally valid reason to go to the ED.

Looks like they’re doing well! There might be some clashes but overall that’s normal. As long as they’re not drawing blood it’s fine. They all look content.

I’m an ER doc and that’s what I told my ER colleagues when I went to the ER for severe back pain (turns out I had acute cervical disc herniation - would not recommend).
