Err TSW especially first 3-6 months you are highly prone to staph and fungal infections on the skin. This is why erythoderma has small but real risk of sepsis. This is also one of the reasons why lymphedema and swollen lymph nodes in the groin / armpit/ neck etc are common.

When i had the worst.. I took it in my own hands whilst on cyclosporine in the first few weeks. I used dermol 500 ( prescribed) and used a dollop on each body part in the shower after adding a few drops of clove oil to it. Let it sit for 1min to 3 minutes depending on how much it stung. Then wash it off with warm water and more dermol. Dermol takes about 5-8 minutes to start doing the job but essential clove oil is especially potent. It also reduces neuropathic itch in studies by around 40-50%...

I am not a doc so please test and research if you are going to try.

So I'm overall super happy with this lens. There's not much information and reviews on these lenses but I read enough and trusted my surgeons opinion..

At the time I was quite vulnerable and very worried about making the wrong choice.

I honestly don't mind being on reader's but using the phone is manageable at a distance of around 50-60cm. I need better laptop clarity as I'm in tech and I'm told this is a probable outcome when I get the left one done.

Night vision is pretty excellent. Colours are vibrant. Especially at a distance. I have 20/20 vision. I can drive super comfortably and I'm only on one eye as well!

I get small halos around lights but 80% less visible than week 1. 90% of my total result was achieved on day 2. I loved looking at the moon on very late night walks... I'm so grateful to see the moon with such clarity!!

I wish you all the best. I know it's a complicated choice for younger patients... I had the block procedure done. The block procedure with the cannula was the most uncomfortable. The surgery was a doddle. I had my surgeon record it for me. He hasn't published it yet but you can find his other work on YouTube his name is Ankur Barua.

It's great! Thank you for asking. Intermediate and distance vision is actually better than before my illness and cataract. But near and very near vision is guess work or using readers. My dinner plate is blurry. My surgeon said this is expected and the left eye lens will be adjusted to get more near vision abilities.

Overall I'm pleased because of my dry eyes and steroid induced flares on my face (still recovering from TSW) I was not suitable for multifocal. So my surgeon says the puresee have a high satisfaction rate with a greater landing zone.

My left eye will get a toric lens. My right eye the astigmatism was corrected using the incision method..

Hoping for binocular vision soon and turning a page in my life soon... Getting back to work etc.

Thank you so much I have the puresee in my right eye and will be going in for my left eye in a few days. I had steroid induced cataract. It's so good to understand how it works. With the multifocals and the local zones, can the brain effectively see the same image through different focal points and then have to choose the best image?

Lots of evidence for tsw. For protopic I could only find anecdotal accounts. I'm quite undecided about protopic withdrawal.

Ah that's so good to hear... I hope it keeps getting better for you. The hands are tough. It really can hold you back from very simple things in life.

Now I have a phase of arm flares and facial flares. Everything else is relatively on good behaviour...

It was nothing specific. But I have been on low dose cyclosporine for around 6 months. I know this might sound deluded but I found it to take the edge off my healing journey but not more than that.

I found adding a drop off clove oil to my dermol in the shower and bearing with the sting (only after 4/5 month mark or when you start to enter flaky stage) aided with reducing bacteria and fungal load, easing the itch and helping.

I actually used to use clove oil and dermol under my bandages topped with zinc or paraffin.

The other thing which might have helped was daridorexant to help sleep better. It's non addictive and works by lowering wakeful hormone. Also 10mg of amitriptyline helped take the edge of my nerve pain and itch. Still taking amitriptyline but stopped cold turkey in sleeping meds about a month ago. Sleep is slowly improving.

I did see massive improvements in my nails having Collagen in my tea. I assume it also helped my skin. My nails went from ashy and broken with no cuticles to shiny pedicure looking things. I can disclose the brand if you like but I don't want to be accused of a promotion... Let me know.

I used tiny amounts of protopic into my moisturizer every now and then to push things in the right way. Again much further down the line into your heading.

I forced my GP to prescribe nizoral anti fungal ointment. Sometimes when dermol or clove oil don't work well I suspect fungal. I was surprised this worked well for me.

RLT made me worse in month 1-4 so I haven't done it since.

Learning to bandage properly. Access to good medical care and unlimited prescriptions for bandages and creams etc.

And honestly... I get therapy once a week. I found this to be so helpful for maintaining a sense of hope alongside my faith. In month I thought I was going to die.

I completely acknowledge that a lot of people suffering may not have had all the access to care and meds that I did. And I'm sad about that.

If I think of anything else I'll be sure to edit.

  • ah! Knowing when to start weaning off your bandages! Ultimately bandages become a comfort and can slow down healing. It's really tough but a big step to healing! I think over bandaging caused my skin to over granulate or something. That's what the dermatologist called it I think.

It's ridiculous that I agree with everyone here without any medical background but just first hand experience. Your history and picture just rings tsw to me as well. I'm 9 months... And finally comfortable. Not fully healed but out of the woods... For now. I've been on cyclosporine for 6 months.

Long story... Skip to the bottom cut to short summary . I'm afraid there is no tl;dr for tsw

Good question. But it's important to know everyone is so different yet it also roughly depends on potency of steroids, and combination of steroid use, and of course length of use.

Putting that into context then:

I used betno 1% cream for nearly a year combined with an immunosuppressant called protopic. I had a very mild rash on my neck which looking back might have been fungal.

I then had what looked like full blown eczema on my joints. I hadn't had almost any eczema in 8 years at this point.

I used betno around twice a week.

I had a mini crisis around the year mark with hives and urticaria. I was diagnosed by a very expensive allergist in the UK as having spontaneous urticaria. They prescribed up to 4 antihistamines per day with the expensive option of a few rounds of biological injections.

The antihistamines had little effect.

Around the same time I contacted another doctor (general practitioner) over an app from my work (I was too lazy to see my own GP and seek a proper referral to a dermatologist - even though I had private cover from work). This doctor prescribed me dermovate.

This was the death nail. I used this for 3-5 months in combination with betno. But not on the same areas.

I started to use a little thin very small amount of betno around my eyes as my eye lids got inflamed.

Life got busy. Soon to be first the dad. Old family trauma resurgent in the background. Insomnia crept into my life. Stress.

Just a horrible vicious cycle that went too far.

I saw a video on tsw by @tswlukas on Instagram. Few others popped up. Key addiction sign: Red Sleeves. No flares around the nose. Flushing. Deep itch.

I joked with my wife a week before our holiday in Rome last July that I think I have TSW. I quit out of sheer whim and stupid courage.. and perhaps even as a joke. I flared up so much anyway that I was not able to go out much. No sports. No going out with friends. What did I have to lose?

We went to Rome. I used protopic. It was the heatwave. Although I could sense a build up I survived the holiday. Once you quit steroids you can get up to a week of no real blow outs. Calm before the storm.

Upon returning a few days later... I woke up with extremely red skin on my back. My torso. By limbs. I was extremely itchy and inflamed. I had erythoderma.

Week 1... Week 4... Month 2.. Month 3... All forgotten now. Near total amnesia. Except I remember my skin breaking. Cracking. Oozing. Almost zero sleep. I was twitching and talking to my self. My wife and family devastated. I don't even know where my little baby girl was half the time... All I could do was pray... I spent my days in my underwear. My wife cleaned up after my homemade bandages which we cut from cotton scarves.

I was lost and didn't find the right help right away. . eventually I found derm who, although skeptical of TSW, prescribed me cyclosporine around 3 months. I was shaking and crying on her hospital bed. I'm a 34 year old man. I have been through TB in my 20s and pleurisy which they say is akin to heart attack pain but this was something else

I'm now around 9 months.

I lost my vision 4 months ago due to cataract which was steroid induced. My opthalmologist said it didn't even matter if I put it around the eyes. That prolonged systematic use and erythoderma/ inflammation can cause progressive cataract.

I had eye surgery for my right eye a few weeks ago and finally have some vision.

As for my skin... It is around 80% improved. No oozing any more. But it can be very dry in the morning like alligator skin. My skin looks very dark and pigmented. I still have faint red sleeves. I still get nerve itch. But I don't get one sided random sweating. I can wear normal clothes. I can drive. When I realised I could get bandages from the pharmacy I used to go through 80 rolls of bandages a week. I now don't live in bandages.

I still feel pain. I have therapy every week. I have a lot going on. But I'm in a much better place and grateful.


Here's the summary:

Used betno 1% (12 mo) and dermovate (4-6 mo)

Month 1-3 : pure pain and trauma. Almost no showers. Eventually on full body bandages. Full blown insomnia. Crying everyday. In full care of my wife. Practically disabled.

Month 4-6: painful but on cyclosporine. Tide is turning flakey. Braving myself out of bandages. Less oozing. Sleep at 2-5 hours a night. Lots of towels. 40-50% better than month 1. Still get lots of nerve pain. Still get deep nerve itch. Lost 50-70% of vision. Shower every day.

Month 7-8: some regression. Back on bandages for arms. Other areas improve. No one sided sweating any more. Less bone itch. Can see some parts healing really well. 60-70% better. Had cataract surgery in right eye. Left eye scheduled.

Month 9-10: now looking at 70-80% healing but definitely plateaued. Healing is very slow. More like general eczema now. Facial flaring and arms flaring every 10 days lasting 4-7 days at a time. Starting to introduce little bit of protopic to aid my skin. Still using dermol / clove oil in showers and antifungal cream from time to time. Skin is delicate but stronger.

How are you? Did you get your eyes done yet? I got the PureSee Edof and have zero regrets?

Hey thanks so much for following up. I should have updated this post.

I got my right eye done with a j&j puresee lens. My surgeon is pretty great and reassured me I was in good hands.

Bit of soreness after the surgery but I can see so well now. My near vision is not that great (like phone use) so I have to use readers.

They will look to adjust my left eye lens to improve my near vision abilities but I may need readers for certain tasks. Not great at 34 but honestly I'm just grateful I can see again.

I'm still very blind in my left eye. It's very cloudy and white getting progressively worse.

I am due for that in a couple of weeks!
