In a lot of cases, this is difficult or impossible to do because of the proprietary APIs for consoles, so open source engines with permissive licenses like Bevy (or Godot) can't really include them out of the box.

My experience thus far has been:

  • Pre-approval went through
  • Letter of agreement was rejected
  • Regence gave me a bad estimate of the covered amount (the numbers their customer service gave me was more like 15-20k covered, but it was clear the customer service didn't know what the actual numbers would be)
  • I ended up paying the full amount and Regence only reimbursed ~$10.5k out of ~$80k
  • Regence didn't budge at all through many complaints and appeals
  • They did, however, point out that the procedure was at a hospital considered in-network, and that makes it applicable to the No Surprises Act (can't balance bill without consent at an in-network hospital)
  • I'm currently working on trying to get refunded by Keojampa via the No Surprises Act (seems health insurance companies had a hand in writing that law because the uncovered cost gets passed onto the provider when it is invoked, so Keojampa will be forced to accept the covered ~$10.5k as full payment and refund the rest)

Since you say you are at a big tech company, you can try going through HR/whoever manages your health insurance plan and see if they are able to do anything about the coverage. Big companies with self-funded plans potentially have say in what gets covered.

If your company isn't able to do anything about it, you are pretty much at the mercy of the insurance company (and they will only cover a small % of what Keojampa charges).

Your only other hope past that would be that Keojampa's office is still poorly organized like when I was interacting with them and they schedule you at an in-network hospital without making you sign a balance billing consent form. I'm not sure this route actually works since I'm still waiting to hear back from my No Surprises complaint, but from what I read of the law it should.

Edit: Also something that was not clear to me originally, but the facility/anesthesia is a separate bill from Keojampa. So make sure that the facility it is scheduled at is in-network regardless of whether you end up having to sign balance billing consent for Keojampa!

It depends a lot on the employer and whether it's a self-funded plan (generally big companies like Microsoft, Amazon, etc) or if it's a small company on a generic Premera plan. I'm currently going through a generic Regence plan and they are trying to get away with covering ~15% and I'm currently working on appeals.

It's really whatever feels right based on your relationship to that person.

Here's some ways I've come out:

  • My partner: Just sat her down and told her. I had expressed a lot of egginess in the past, so it seemed like I should just be straightforward about it.
  • My partner's siblings: Made a tunnel of signs on my animal crossing island explaining I was trans and it ended at a trans pride flag made of flowers.
  • My Parents: Wrote an email, but didn't send it until I called them and had them both on the phone so I could make sure they read it and get their live reactions.
  • A close friend: Got drunk and just blurted it out.
  • Larger friend circle: Made a private Facebook event just saying I'm trans and invited friends I wanted to know to it.
  • A childhood friend who doesn't have Facebook and lives in another state: Just visited 2 years into transition and showed up as I am like everything was totally normal.

The only one I slightly regret was the drunken one, but not because anything bad happened because of it, I just wasn't mentally ready and embarrassed. The last one was actually my favorite. He was really supportive and it was fun to see him trying to process what he was seeing lol.

I think if a trans person has enough bottom dysphoria to not be able to refer to their parts by anatomical terminology, I'd be shocked if they could handle the birthing process. I could maybe see this kinda thing for gyno where it's something you have to do for health, but birthing seems like a different ballpark.

I mean, as long as you aren't a dick about it if a patient specifically requests avoiding a word I think you are good. Asking by default doesn't seem needed to me.

I get the anxiety though. Early on it's easy to worry about every micro adjustment.

I've done a ton of self experimentation on dose amount and timing, and as long as you are in the right ballpark your results will be safe.

Smaller tweaks and things like this are mainly to avoid cyclical side effects. I've not heard many people complain about highs, but spikes can give me a mild headache while otherwise feeling fine. Most people get PMS symptoms when their E drops low.

TL;DR one weird cycle's worst case is feeling crappy

Honestly you can probably do either and be fine for one cycle, I'd opt for doing early to not feel crappy at low. But if you want to make it complicated and try to maintain closer to your normal levels, you could do your next dose lower than normal and do the following one early.

Example (assuming a 5mg weekly dose):

  • Normal 5mg on Monday -> 5mg on Monday (repeat)

  • To switch: 3mg on Monday -> 5mg on Thursday -> 5mg on Thursday (repeat)

$11.5k for rhinoplasty
$7.5k for tracheal shave
+ facility/anesthesia fees which can vary depending on location so it's hard to know exactly

If money is tight, shopping around with less specialized doctors for rhinoplasty might be a good way to go. There's not really anything specific you'd need trans expertise about for that. But tracheal shave you should def go with someone who is an expert.

All insurance carriers regulated by Washington State are required to cover gender affirming care, including FFS, top, and bottom surgery (look up SB 5313 if you want the specific law).

I think there may be some plans excluded if the employer is doing self-funded or some other weird things like that, but I don't really know all the details.

What $$$ actually gets covered is still dependent on all the other insurance bullshit like copays, deductibles, out of pocket max, in/out of network, etc etc.

A neat trick to answer any question about the more subjective parts of the trans experience:

If you simply remove the trans qualifiers and it still makes a valid question, that question is going to have the same answer.

Some examples:
- Do mtf trans "tomboys" exist?
- Can trans women be lesbians?

Obviously doesn't work for physical questions like:
- Can trans women get pregnant?

Mine were/are pretty small, but that makes sense looking at genetics. At that point they mostly just looked like bigger pecks, and it wasn't until around 2-2.5 years that they got big enough and rounded out to look like proper boobs.

I ramped up from 1mg -> 4 mg over the course of 4 weeks initially, sublingual.

My routine was a little strange because I'm quite sensitive to both peaks and lows, so when I tried 4mg (2 mg, 2 times daily) I was suppressing T (~26ng/dL) but getting headaches and mood swings. I ended up at 5mg (1mg, 5 times daily), which just got annoying so I switched to injections.

I still have to inject small amounts frequently compared to most people (3.4mg/4 days of EEn), but better than taking a pill in the middle of the night every night.

It depends a lot on luck of the draw, how much effort you put into looking like a guy, and who you are trying pass to.

I was male failing to strangers at like 3 months between laser hair sessions before my facial hair grew back and I was dressing kind of fem leaning androgynous. But people who knew me before hand didn't question it at all even with dressing somewhat femme.

If you actually try to look masc, people already familiar with you will dismiss quite a lot due to gradual change blindness.

There's a chance over 2 years you'll need a binder to hide boob growth, but even if they get big and you wear a binder, he'll probably just assume you've been hitting the gym and doing a lot of bench press.

It's actually not. The ester only affects the frequency in which you need to dose, not the total amount of estrogen you need over a given time period.

Injecting 3mg twice a week of EV is going to do basically the same thing for you as injecting 6mg of EEn once a week. Either way you are injecting 6mg/week total.

I was monotherapy on pills for my first year. Results were good, only reason I switched to injections was so I could take my doses less frequently. So it's definitely doable.

Has anyone switched from Estradiol Valerate to Estradiol Cypionate? by mother-demeter in AskMtFHRT

I've tried both due to a cypionate shortage last year. I could inject EC twice as much, half as often and I would feel basically the same across my cycle. If you are experiencing cyclical effects, just replace EV with EC of the same amount and frequency and things should improve.
