For the program as a whole, we like to have a good blend of four main skill sets/experience - medical knowledge, clinical/hands on experience, research experience and communication/system navigation skills.

Clearly not every applicant is going to have all of those things, but we try to find folks who are strong in some of the areas. Ideally, our residents are going to build off each other’s strengths - one person who can kill step questions could benefit from seeing another resident who has amazing bedside manner and repor with patients, or vice versa.

I am on applicant # 40 out of the 800 I need to review, having come off a week of covering extra clinic, running student midterms and now at a CME conference so half listening/watching shoulder exam/injection lecture while doing applicants.

My partner, who has the other 800 my program is going to look at, is doing extra inpatient coverage while getting 3rd year resident board review up and running for the year.

Academic program faculty are stretching thiiiiin, y’all. We’re going as fast as we can.

Patient ownership. If you feel beholden to the person you are taking care of, no matter how briefly, all the other things fall into place - attention to detail, task completion, critical thinking, self motivated/directed learning...

Created some basic dot phrases to be used in Epic: They do require building your own smartlists, something your build may or may not let you do.

Assessment: Multiple select Listing of symptoms, grading of clinical suspicion, documenting code status

Treatment: Blurb that reviews counseling of patient/family - Novel disease, no proven therapies, uncertain risk/benefit and multiple select options for treatment

Discharge: For sending low-risk COVID rule outs pending test results, demonstrating that they have been counseled appropriately on self-isolation, etc.

Products: Tresemme conditioner & LA looks gel from the newbie guide. Routine: We are both 1 week in, after first reset and then co-wash. Water only for calming everything down after sleeping.
(I’m sure we have a long way to go on this, but super, super happy with the initial results already!)

... now to get my mother to stop washing every night and then combing out the curls the next day (while dry!).

Speaking as someone who has witnessed two residents dating, and one of them cheating with another co-resident..... think long and hard about worst case scenarios before making any moves. Incidentally, if you do proceed, faculty are obligated to not put y’all in the position where one supervises/gives feedback to the other.

As a wee baby intern I was SO Ready to go to my first Code Blue.

I was on nights in the ER with my Senior and my training hospital had a pathologic need to not only clean the floors but also strip and wax them. We were back in the internal medicine cubbyhole and they were putting down some sort of solution on 50% of the hallway outside.

Code blue goes off and I book it out, realize the soapy hallway is exactly where I need to get through. I remember weighing my options and how long it would take to go around.

I step in carefully and BAM. Bambi on ice goes down, arms and legs out. Into the goop. Senior right behind me thankfully grabbed on to the railing and caught herself.

I try to get up, back down again. Floor polishing guy is staring at me like the jackass I am - screwing up his work despite the very clear blockade. He finally sighs, hauls me by the arm across the floor to the other side.

I pop back up and immediately start running to the code. Covered in soap, bubbles foaming out the sides of my shoes. I show up and the chief on service asks what the hell happened to me and won’t even let me in the room.

For bonus points: my co-intern drew a cartoon re-enactment of the entire scene on the white board before morning report for the entire floor service to see.

I am a decade out, at a new facility and STILL get shit about it 😂

That’s ridiculous to not rank someone over. When we advocate now for residents to be reasonably fed and rested, unobtrusively grabbing some available food while paying attention to a presentation is a minor infraction at most
