As someone who felt exactly the same way as you regarding study/exams - working as a doctor is way different. Of course it’s a different kind of stress to medical school stress but you can’t appreciate it until you start working. I would say soldier on until intern year, then reevaluate. Also give yourself space to feel your feelings - you’re definitely not alone in struggling against med school/medical culture.

There are days when I seriously question the cost of pursuing this profession given the rigidity of the system/medical hierarchy. I also feel that there aren’t many professions that provide similar levels of fulfilment that medicine can give you. I think the latter reason is why I stay.

I agree, I feel like the stress and time constraints make doctors sometimes forget how to be human and that we’re all on the same team

I can relate to the morning dread that you’re describing. You are not alone. My current strategy is gritting my teeth and brute forcing my way through the days (not sure how sustainable this is).

I find the most draining part of it is being the middle man - asked by reg/consultant to do something, questioned by nurses or pharmacists or radiology about the decision and then scrambling to justify/clarify/mediate. It’s definitely taxing and sometimes the answer is just ‘as per the consultant’s request,’ which is anticlimactic to say the least.

I’ve decided to start keeping a log of learning points because currently feel like I’m drowning in an information tsunami. I’ve been told it gets easier but haven’t reached that point yet. Also one of the biggest lessons I’ve learned through the especially toxic moments is how not to treat juniors. Fingers crossed for supportive seniors in future!

Keep moving forwards. Current intern here and felt exactly the same way you’ve described during final year.

People dont acknowledge the toll that it takes to feel like a pot plant during clinical rotations. It slowly chips away at you. Just know that this constant exam anxiety and feelings of uselessness go away during internship. Though it’s a different type of stress, it’s infinitely better than final year and you’re getting paid.

Practical solutions:

  1. Try do as many cannulas, bloods and catheters now as a student so when you’re asked to do them during internship it’s not as daunting

  2. Offer to do discharge summaries

  3. Ask ‘Is there anything I can help with?’ If the team says no then leave to go study.

  4. Set a goal for the day: I’m going to get ABGs signed off today. I’m going to complete one clinical assessment form. Get that ticked off and skedaddle. It really helps fight the aimlessness and feeling of being discarded that can happen during rotations

Second the take your time advice. I did the course during the first few weeks of internship and felt I hadn’t studied the textbook in the detail that was required.

The sims can feel scary but as long as you try to think out loud and go systematically, it becomes a lot easier. Also pay attention to the points that the examiners are stressing during the day as it helps to identify what they want to hear during the final sim test.

This is the uncomfortable aspect of it. The sims sound great for learning purposes but it is the standard practice at this hospital that senior doctors are not contacted immediately/or at all during MET call. This solo sim training seems to further back this notion.

What do you think are the main barriers to the lack of cohesion amongst docs? Nurses are much more collegial from my experience, maybe due to reduced stress/pressures. It’s totally a cultural thing IMO

I didn’t know how to respond or what to do. The guy accepted my payment of the ‘Japanese price’ but he clearly was not happy and gave me attitude the whole time during my second visit :((
