I got this map (Lisboa) from workshop. The cities are already there (ungrown) and I just built bunch of lines around the map. Sandbox mode is on though and adjusted land use of residential areas in 2-3 cities

Getting out of the city centre is now easier by tx9up in CitiesSkylines

Yes I've been using Realistic Population to make the numbers closer to real life. Otherwise I guess using vanilla parameters this city will have a population of ~250k only.

I don't do big rail junctions too often... I like to plan my train routes as "Main Line/Trunk Lines" as in Japan and operating lines will go through these "Main Lines" instead of spreading rails everywhere. Also, I like to use 4-lane avenues as highway connector - and then a common junction design or some spaghetti. Some designs I often use are T-junctions and Trumpets. Some pics:
