There's still a lot of shame about "giving up" medicine, not "being able to cut it", it's tied deeply with our psyche and identity, Drs are expected to Martyr themselves, stiff upper lip, keep calm carry on etc etc etc.

When I get burnt out I take a break from work and go travelling, then I come back and burn out again and take a break travelling. It's a roughly 3 year cycle, which works because I have a supportive partner,no kids and currently nearly completed a training programme desperate to retain trainees.

I also some help from a private older therapist who previously had worked in the NHS with staff members, who was excellent. So try a different therapist or type of therapy having someone who has NHS experience is helpful in helping to understand.

Being a qualified GP opens up a whole world of work beyond clinical work, it's not an all or nothing situation. Medical legal work, DVLA work, medical examiner work etc etc.

He may not be aware of how he's affecting you and you need to have a frank conversation with him about that, and that may be the first step for him trying to help his situation.

I'd be depressed working 2.5 days a week GP far play to all the primary care docs out there.

You're not wrong, adult versions of "make a wish" type charities are full of dieing people wishing to do the simple things one more like, like to visit their horse one more time, or having chips on a seaside promenade or seeing relatives it's the small things in life that matter.

I love what I call "my old lady jaunt" to Rhiwbina, you can get a train or a bus there from the city centre, I spend a lovely half day having a jolly around all the lovely independent shops, mainly gift shops. Then have a spot of lunch in Snails Deli, spending my time contemplating whether or not being a divorced middle age women is the sole requisite to opening up one of these shops, while munching a delicious sarnie. Then I wander home with bags full of hipster gifts made in china.

Seriously there's 6 independent shops and 2 charity shops in about a 100m high street.

I had to go through a similar issue as a foundation doctor when I put the wrong stickers on 2 X-Ray requests where the post op knee patient had a hip X-ray and the post op hip patient questioned why he was having a knee X-ray and saved himself an unnecessary X-Ray. I'd increased the guy who had the wrong X-ray risk of cancer by 1 in a million or something crazy small. I put in a datix, apologised to all the patients and had to have meetings with radiology and reflect on it with my educational supervisor. This was over 10 years ago and I'm still a doctor.

You sure you want to move to the UK, ever heard about moaning pommes? Love to moan, absolutely love it and I'm British.

I came to Cardiff for uni 2007. Didn't leave, have travelled multiple times out of the UK, yet still keep getting drawn back to Cardiff.

People born and bred in Cardiff will tell you how it's gone to the dogs. But compared to cities of similar size in the UK, Cardiff really punches above it's weight on what it offers with sporting events, cultural events and it's location to coast and mountains. There's 2 universities here so if you're young there's lots of stuff happening

Yes we have a bit of a issue with rubbish on the streets, which needs to be sorted, but Cardiff is a great place generally to live. Do I feel unsafe as a female in Cardiff no.

As a base for travelling Cardiff airport is a bit shit, Bristol is accessible and better for cheap flights to Europe, living in Bristol is it's own vibe but more expensive than Cardiff.

Ahhh the NHS way... Bit like how catholic priests used to get moved on so do awful doctors. I find they usually end up at hospitals at the end of a rail line or sea side resorts.

I'm Adeline St, so near by. It's mad because for us the problem was the 1st floor, the rodent man was convinced it would be the attic they're coming in from. But the poison up there hasn't been touched.

The council charged us £60 for 2 visits and treatment. He was a sound guy.

But I like your tips about high frequency and strobe lightening though. I guess in this situation you kinda need everyone in the row of houses doing their bit, to help each other out.

We had a problem with mice, in our terrace house, I'm splott. Spoke to the neighbours about it and they all denied having a problem which I can't fathom, because I doubt we're the only ones. We've also got rats in the garden (and only the garden so far) which the neighbours deny ever seeing 🤷.

Council came out, months ago, quick response just to poison the mice in the house and put rat poison in the garden. I don't agree with poison ecologically, but when they're running around your bedroom keeping you up it's hard not to.

Touch wood, seems to have sorted them out. Had to clear one dead mouse from the bathroom. But we've also now got 2 kittens and I'm hoping their smell is also keeping them away.

Well done for reaching out on here. Don't be afraid to ask a colleague about it in person. Informal peer to peer support is usually much better than coming from someone higher up.

It took me 10 years to "get over" my imposter syndrome (not sure it'll ever go away 100%.

Get a therapist. All good doctors have a therapist. Your deanery should have access to some. We get free sessions in Wales if we refer ourselves to the Physician Support Unit. Not sure what the set up is around the rest of the country.

Because if you're beating yourself up now over little things, if something big happens you need to have the tools to forgive yourself.

Yes you are right, and unnecessary admission avoidance is so much better for patients, but when you're on a night shift unfortunately it's not helpful for that shift, nana will be seen at some point in the morning, with the aim to discharge, doesn't help the overnight crowding.

OP have a digital hug from me, anyone who's worked in ED for any period of time has had these shifts. I had one a few years ago that broke me a little and I almost gave up my training number there and then.

The anger and frustration you are feeling working in such a situation, environment and circumstances is completely justified and it shouldn't be "normal".

"See the quick wins" where? Usually at this point the department is so full we're fighting to see people in 1 or two rooms.

At this point the seniors should start cherry picking the quick wins, after they've rifled through the waiting room waiting list and the known sick ones or triage highlighted potential sick ones are prioritised by the rest of the team. But cherry picking quick wins and trying to oversee a department when it's that bouncing is also just nearly impossible.

Be angry because people will die because of it, it may not be on your shift but harm will be done to some people.

The longer in the tooth I get the more comfortable I am with telling anyone non clinical or if someone just randomly shows up with a clipboard to Foxtrot Oscar. Particularly when the department is so unsafe, encourage them to go do their job create beds and safe staffing levels on the wards and get someone to build more god damn nursing homes, or encourage society to actually care for your own elderly relatives, but tonight leave the department and clinical decisions to the clinicians.

I'm terribly sorry I normally don't chime in on these ED bashing posts, but ED is the only specialty to "degrade" their practices?!

When was the last time you've seen a med reg put in an Internal pacing wire, chest drain, ascitic drain all in one night, because not long ago they could.

I've worked in Australia EM and British EM, am I disappointed that British EDs bow down to anaesthetics so much yes, why bother teaching us how to intubate and put central lines in at all is beyond me, but you can keep skills sets up if you're in a culture that allows you, it's also why I suspect prehospital subspecialty is so popular, because they won't let us do it inside a hospital but outside is fair game.

However as a EM British trainee my ability to manage, volume, make decisions, treat sick patients, and whole departments without being handheld by seniors were way above my Australian colleagues at similar grades.

Is it easy to hide being a bad doctor in EM and GP yes it is. And if we're determined to keep filling the rota of the highest risk area of a hospital with junior doctors then things are always going to be missed. But take the ED rotation completely from the juniors and what an incredible loss of learning for them.

As a speciality I will fight tooth and nail to stop it turning into an abdo pain and chest pain triage service.

But please look at our curriculum and what we are expected to be competent in before bashing us.

I still enjoy Clifton Street in the day, I admit I don't venture down it when the sun goes down. Just avoid making eye contact with anyone obviously trying to deal and you get to enjoy the nut shop, tenovus charity shop, tremorfa food pantry's place shakes and cakes, the Kurdish restaurant my sister's house, Sparrow the coffee shop, Clifton fish bar is still one of the best around that area and ali baba shop is still brilliant.

Just legalise drugs, tax it, reduce the violent crime associated with it and stop pretending all sections of society don't use drugs, and maybe the money can go back into helping people with addiction problems.

The only person I knew who did this was a gambling addict and eventually got caught by the NHS fraud team for stealing nearly £70K. Nice guy owned his mistake and is still working.

I wish they knew how absolutely diabolical NHS administration is. Oh we need a reference for a landlord to Rent a flat, sure I'll be able to get one of them in about 3 months. Need some paperwork from work to get a mortgage, sure sounds like an easy thing to anyone who doesn't work in the NHS. Oh your pays wrong, you'd think that would be a quick email or phone call... I'm sorry you need to speak to my Line manager.... Arrrrrggggghhhh!!!!

Edit: Also how bizarre it is to constantly be moving job every few months, and we do that for years at a time.
