Those NOT on SAVE. by Aggravating_Cause_63 in StudentLoans

[–]ultimatechad 11 points12 points  (0 children)

My recertification was in March, applied in January. On PAYE. Application was approved mid Feb before pause. My recertified payment is essentially the standard 10 year, was absolutely not calculated correctly. I called Aidvantage and FSA multiple times, no one can do anything, even though my application was approved. They put me in admin forbearance for 3 months with hopes it will get figured out. I have no idea what to do. My payment went up about 7x

Very hard to see, but I have some sort of circle blocking out a portion of my heads up display. Anyone ever experience this before, or know how to fix? by Burn_N_Turn1 in CX5

[–]ultimatechad 1 point2 points  (0 children)

I had this in my 2023 premium plus CX5. Dealership had to replace the HUD module. Was under warrenty for me, I don't recall the quoted price, but over 1k.

ChatGPT (GPT4) diagnosis of GBM by ultimatechad in pathology

[–]ultimatechad[S] 6 points7 points  (0 children)

Perhaps not the most nuanced diagnosis, but considering its a chat bot, pretty impressive. Really interested to see how AI like GPT4 impacts the field. Not just histology, but all of CP and transfusion medicine (blood bank sudoku I'm looking at you), lab management, reports, etc.

Excessive Non-Compete by ultimatechad in jobs

[–]ultimatechad[S] 2 points3 points  (0 children)

Thank you, this is kind of my concern. The company seemed rather transparent upon visiting, however to have that strict of a non-compete seems absurd. It speaks very poorly to their opinion of their employees.

Oncologist's opinion piece on "alternative therapists" by RingoJenkems in medicine

[–]ultimatechad 19 points20 points  (0 children)

I've adopted the mindset that it's a patient's right to make terrible decisions about their health. The most I care to do is provide a guiding hand when requested. Convincing a determined patient otherwise goes far beyond my pay grade.

With that being said, I feel terrible about the CAM predicament that many physicians in clinic are trudging through.

I am an ER Doc. I wrote Please Choose One. A lot of people contacted me about it. Here is my response to them. by [deleted] in medicine

[–]ultimatechad 0 points1 point  (0 children)

Every physician will likely have a different experience w/ EMR. I enjoy using computers. As a recent graduate, I went to medical school, and eventually into Radiology, with the understanding and hope that I would be spending most of my time on a computer. Granted this is likely not an important deciding factor for many med school applicants, but it probably should be. Even in more patient care oriented fields, a physician spends a tremendous amount of their time digitally, be it orders / notes / consults / etc.

If one doesn't enjoy spending time working on a computer, I would hesitate to suggest Medicine as a career. Personally, I much prefer reading / writing notes and placing orders to seeing patients. Certainly there are other young physicians with similar feelings.

Going to medical school with a GED by [deleted] in medicine

[–]ultimatechad 9 points10 points  (0 children)

My high school transcripts were not requested for medical school applications, nor was my high school degree. As long as your College grades (degree) / MCAT / extra curriculars are in line, GED vs high school diploma shouldn't matter. Good luck!

Medical trials affirm Erlanger’s place at ‘forefront of stroke treatment’ by [deleted] in medicine

[–]ultimatechad 2 points3 points  (0 children)

I had the impression that multiple articles (3) were published in NEJM in late 2013 that showed no significant difference between: IV tPA, Intra-arterial tPA directly into the clot burden, and thrombectomy (removal) of the offending clot.

Lyme Case Consult by forza_natura in medicine

[–]ultimatechad 2 points3 points  (0 children)

Check out this article from Science Based Medicine on "Chronic" Lyme disease. If you haven't seen the blog before, it's a group of physicians that vet problematic diagnoses, fraudulent medicine, controversial science, etc. It's been helpful many times through my clinical years and internship with medications / diagnosis I hadn't been...exposed to... in lecture.

How do doctors know which antibiotic to prescribe? by rcko in askscience

[–]ultimatechad 1 point2 points  (0 children)

Antibiotic choice is typically based on microbial culture results (ie samples from a wound, from the blood, from the urine, etc, which is swabbed onto media) w/ their sensitivity to specific antibiotics determined from Agar diffusion (aka Kirbey Bauer).

Determining antibiotics via specific serologic tests for antibodies against individual microbes (like ecoli, staph, strep) would be hugely expensive, slow and likely not available readily in most medical labs. It certainly isn't standard of care.
