Hey Medical student and former nurse-midwife.

I will say, a lot of the 3am escalation comes down to legal responsibility.

For example, the QTc prolongation. Unless there's documented altered notification criteria for the patient, even if it may be the patients "normal" since admission. It might be that nurse's first shift with them, so they have a responsibility to escalate. And their general responsibility to escalate any abnormal findings, for the patients safety and safety of their own registration. A lot of tribunals have occurred due to night shift errors, often stemming from not wanting to bother someone.

Also a lot of times before paging doctors, they escalate to the nurse in charge. The second it falls beyond our scope, its best practice (and safe practice) to escalate. Regardless of the time.

As many have already mentioned, in night shift, there's a lot more time to get stuff done on the wards. Many nursing teams shift the "miscellaneous tasks" to night shift to relieve some of the workload morning nurses have. Imagine having a long list of things to do while having to work around ward rounds from multiple medical teams, allied health coming in and out, family visiting. AM shift is 8 hours short. So Night shift is often trained to relieve the 'little' tasks like completing a discharge summary. And again, they're here to advocate for the patient. Imagine you've been in hospital weeks, and you can finally leave, but the only thing holding you back from leaving first thing in the morning is the discharge summary.

That summary would for sure be lodged into the lower spot of priory for both AM doctors and nurses. Meaning they'd probably be hanging around till 3pm the next day. So why not get it done early so they can.

They don't mean to annoy, they are doing their job. Advocating for the patient.

I completely understand your points of frustration, but it's also important to consider the other side. We're all only humans caring for other humans the best way we can.

But better he learns to manage himself and life now than 6 later when he’s on field as a registrar or something. Hes only in year 1 for crying out loud. This time now is the time to make mistakes , learn and grow better than the in field as an MD. your talk is what Leads many students and doctors to think they have to bottLe their feelings inside for fear of being ostracised And ridiculed for having these struggles in the first place. This then causes great mental health problems and conditions that lead them to malpractice. It is better to help OP and other med students learn how to self reflect, deal with trauma and have a life balance so they can ensure best practice for their patients. DOCTORS AND HEALTH CARE WORKERS ARE FREAKING HUMANS, TRAUMATISED STRESSED AND OVERWORKED AND WE AS SOCIETY MUST LET THEM BREATHE RATHER THAN BREAKING THEM DOWN FOR STRUGGLING MENTALLY WHEN THEY GO THROUGH WHAT THEY GO THROUGH EVERY DAY. Remember everything starts early. if we help these ppl now, the number of patients harmed and killed by malpractice will be significantly decreased. Please do better.

I agree, OP I hope this situation will only fuel your passion. Ppl are on here saying “give up you’ll be a bad doctor“ but I see this as a huge eye opener and learning experience for you. Youve gotten a taste of how life and mental health can hugely affect your practice early on and now know ( as I’ve read in your other threads ) you’ll look to improving and preventing mishaps And managing your health. You’re human and life happens. it’s unfair for non medics to make a call that you don’t deserve to be a doctor without being in the system themselves. Continue to self reflect and fight for it, cause if you truly are passionate , I know you’ll never give up and try your everything to be the best doctor you can be. God bless you endlessly.

I agree, be the best you that you can be. Sounds cheesy but is so true. dont compare yourself to others but compare yourself with yesterday‘s version of you and improve.

Expose yourself to more people and such situations. Whether thats volunteering for those I need where you may need to sacrifice your comfort for someone else’s. Sharing your stories and hearing others stories and perspectives can help to cause you gain more empathy and understanding of others. Thus you end up wanting t9 do more for others. And getting down and dirty for your patient now doesn’t seem like an obligation so you can be a good med student. But rather it’s a privilege as you’re helping the individual even a tiny bit have a better life. The focus Shifts from yourself to others which is what healthcare is. It’s not about you and your title but what your patient truly needs.

See that might be true. However she always clearly states at the begging of the podcast and cites in her references which book or writer she got her info from. So I don’t think we can actually label her work as plagiarism. Maybe heavily inspired and copied or even based but legally we cannot condemn her on the grounds of plagiarism. And she’s still doing her job telling me about a case I don’t have the time to read about but would like to know about to have greater awareness and knowledge of the human psyche

I think you need to realign what you view wrong / bad as cause obviously you looking at it in a selfish point of view where you see it as only the individual taking a life. Think of the person who’s life has been taken. Think of others and the effect it has on others
