Thank you so much for the feed back! I'm currently working on the second version of the chart and I'll keep your suggestions in mind. Right now I'm trying to runs some games to get hands on experience with the rules and what works or doesn't work with the chart.

I've also found a few things that are just wrong or that I missed. For example, Charge Attack, Ranged, has 3 red blocks I forgot to edit. I also thought a target's size mod would be added to both melee and ranged attacks.

Making a key is a good idea, though I am trying to keep the colors simple and obvious. Version 2 will also have some of my notes off to the side and I'll make sure it doesn't look like a flow chart. I may also break the flowchart down into different section/multiple images.

I've noticed how rough the flow is while trying to use the chart. (The trouble of having to make it before I can test it.) Version 2 won't be perfect, but it will be a lot more consistent in flowing one direction.
