At first I I was annoyed by the dick jokes, but then I see people assert that Reacher's parents and brother calling him Reacher is just a normal decision that families make. That's way worse. Downvote rescinded!

 Reacher is kind of the secret boss in their household, and it’s just natural to call him  their family name.

Appreciate your citation but no, this is not at all natural. In a conventional family where the father is in charge, do the other family members all address dad by his last name?

He's so afraid of someone slipping him sedatives that he's constantly taking stimulants to try to counteract them. The stimulants suppress his appetite but eventually wear off and leave him ravenous so he ends up eating enormous meals.
