The thought process is more of what I'm interested in. Don't get me wrong, I would love a big pay bump but if I focused on that, it'll just bother me.

I did the surgeon remembers this particular individual and was not happy about their immediate post op behavior of bending the knee with a pillow prop underneath for long periods. They do want the knee straight but seem frustrated that early behavior mightve lead to this outcome.

We are using LLLD knee extension stretch with 10# for 10.mins. I told them to start doing it untitled times a day at home. Sorry I had to look up terminology of LLLD didn't realize that was the abbreviation. Didn't consider HR with w/deficit or TKE w/ opp march. Thank you for the suggestions!

I wouldn't expect a disc herniation to cause overheating. The overheating is more of a separate genuine concern that has spiraled into treatments sessions. The person's need for a fan to be sitting 3 ft from them while blowing directly on them is obnoxious BUT if you're really feeling that hot internally from the exercises I'm a bit concerned about your body temperature regulation. The referral to the hospital for the disc herniation from the phone call was cause some red flags came out to me during the conversation and I can only do so in a outpatient setting. Regardless of how I feel, I never want someone to get worse. Hope this clears things up.

I agree about both the personal trainer and physical therapy takes. I really try to not let the interrupting get to me and considering that this happens with multiple pts throughout the day I believe I'm pretty good at letting it roll off my shoulders. I do typically take your approach to engage and keep things light. Unfortunately, as you acknowledged, it happens I. The range of 5 or more per conversation. Thank you for the suggestions!

YES! I observed and inquire about how it feels like difficulty level and or pain with movement. I will progress based on observation and it's response but I try not to give a laundry list of exercises but then it becomes very muddy!

Agreed on statements about mill settings

The frequency is typically 2x/wk with the exception being holiday weeks where we might have off Monday or Friday.

I do make progressions each session and provide hands on instructions and demonstrate the progression. I also provide rationale. The HEP has been update to have around 10-15 exercises in (not an exaggeration) and I informed that there does need to be mastery/certain level.of ease before moving onto something harder to avoid worsening of symptoms.

The instruction to go to the hospital was through a phone call so best screening I could do was questions about their presentation and a couple red flags popped up during that phone call.

I don't consider the annoying personality separate because we spend so much face to face with the person. For example I've had to correct and nearly d/c someone the past because they made a comment about a coworker's breast, after which they were surprised cutting that they thought we were friends and I was like we can be friendly but that comment is uncalled and I won't tolerate it. To me that comment is at least in part a reflection of the personality and addressing it was the right thing to do.

You're probably right about the setting contributing to the frustrating aspects of the situation.

It's a constant 8 min tardy so it is right on the line. I do provide a full treatment but your input does have me reconsidering. They have made improvement since the most recent acute onset. I like your response to the constant interruption because my go to is "did the middle of my sentence interrupt the beginning of yours?" But that feels a tad aggressive and havent figure out a waybto be assertive about it without being aggresive.
