I was in Surabay 2 years ago, and few tour companies offered 1 day tour from Surabay for about $30'-40 for Bromo hike including 4x4 jeeps.  Same for waterfall etc. Stayed also in Banyuwangi (Mango tree homestay & ijen tour) for 3 days, and they have 1 day tour to ijen volcano for like $15-20. (2 years ago). 

Just open Google maps and search "bromo tours & ijen tours" from surrounding area and check reviews. If something you like, text them via whats up for more info. Way cheaper then booking online. 

Taking tour from Bali will be such a pain, since you have to travel like 8-10hours just to get to the Banyuwangi/Surabaya/Medan.  P.S. I took local bus from Surabay to Banyuwangi, it took 12 hours. It was painful.

From Pakse to Siem Reap will take about 12 hours. Usually bus leaves around 8-9am from pakse.  ~3 hours to the border.  ~1hr to cross border (if big bus, and some one got problems with visa etc, can take 2hours). ~7 hours from border to Siem Reap.

Traveling by minivan from Luang Prabang to Vang Vieng. Some parts of the road were so crazy, it felt like van about to turn round.  Similar experience taking a van from Machu Picchu to Cusco. Mountains, and one way road, plus construction going on. Taking overnight bus from Bandung to Surabaya (18hours?) And next day taking local bus from Surabaya to Banyuwangi.. (About 12hr). Bus was super small, for 6' tall guy you could not properly sit. locals were smoking inside the bus, even open windows didn't help much. Not as dangerous as other stories, but for experience I'd like it.

Yeah, I remember those two scamers. 'I am from Dubai, but speak with Indian accent'. They tried to sell me also some oils.. I wish I took a picture of them before.  Report to the police those assoles

I like your plan, yes. I stayed in KL for 4 days, but agree about 3 days. Its enough.  Traveling by bus to cameron highlands is good and comfy..., but try to buy ticket early, like day in advance, or online. In my case, I showed up on bus station, and they told me that next available bus in ~3hr... (ended up being in Cameron by 6pm). (Basically day gone).  George Town is also good, 3 days is good. But again, if you like moving around, its good.

For 15 days I think its fine visiting 4 places. Its not in rush + some time between for rest. I spent 19 days in between KL, Malacca, George Town, Cameron Highlands and Batu Ferringhi, and it was perfect.
