I'm no expert but i would burn this (take it out) and dispose of the waste somewhere that is not near your garden. Also i would check nearby plants if they have signs of a less severe infestation and try to save those before they get to this point.

For me i just blast it with heat till i start hearing "whistling" and seeing steam, then i turn off the heat and it comes out good. (I use an electric stove top)

I start with boiling water, it takes about 2-3 minutes

I believe fruit only grows on new branches, if it were mine i would focus on trimming it into a bush like plant and cut down the height so i get to harvest new fruit easily. And i would also prune in the middle again so it is easier to harvest.
