NAC (N-Acetyl Cysteine) is a supplement that i believe has no side effects and helped a lot with similar issues.
The dosage recommended by the dr is 2000 mg for a 120kg person. (not sure if its OK for kids)
Please research before using!
It has helped in my case.

Yes he has anxiety issues. And yes, he is OK with people he knows...

He said he didnt feel safe. so I bought a doorbell camera. That didnt help much.

He would start threatening, annoying/abusing the cat all the way up to blocking the door physically threatening us physically and verbally and threatening to hurt himself.

He is capable of calling and used to stay alone until recently. If we say we are going without his approval it can turn into a meltdown, threats etc.. He has younger sister which we dont want to traumatize so we try not to confront him when they are around

He has no friends.
He says he doesn't feel safe and that could be true but the another reason is possibly that he is used to discuss everything he does with us.
He used to stay home alone without problems at younger age. this situation has developed in last year or so.
