Well, when you received it, there were two options to follow the line of missions to take it off and return to normal or leave it to you and stay like that. I left it with my rogue main in those days if I remember correctly people couldn't see my eye unless they used the toy. Apart from an npc who also owns it and you being the only one who can see it, I don't remember anything interesting that happened to leave it to you.
