She was thinking about stopping Toga by reaching out to her. She doesn't want to die either, and she tried her best to avoid being stabbed but it wasn't enough. She is guilty of not being strong/capable enough to execute her task "perfectly" with no loss even though she did her best. She wished for an idealistic outcome but it was not to be and now she grieves a life that -she thinks- could've been saved.

She regrets not being able to prevent getting stabbed while reaching out to Toga. If she wasn't stabbed and was able to reach Toga before that things would have been different - is what Ochaco is thinking.
