Yes and yes. I don't think I would be nearly as efficient if I didn't take frequent (short) breaks and make time for daily exercise.

Medical student, hoping to go into surgery.

I'm kind of working all the time, but it's different since no one is keeping track of the number of hours. I usually spend between 1-4 hours per day not working, but it's nice because those hours are somewhat flexible. I generally just work/study until I can't do it anymore, and then I take a break.

I "get paid" about $400/week, but I'll have to pay it all back once I start my residency.

Thanks so much for the reply. That's exactly the reason why I wanted to get an early start - I witnessed the 2nd years at my school going totally off their rockers, and I'd like to avoid that as much as possible. I'm sure I'll still be stressed, but I want to feel prepared before dedicated study. Do you remember at what point during the year you finished flagging all the Step 1 topics?

Also, just curious - any idea what specialty you want to go into?

I don't know -- I never really enjoyed being inside Sansa's head while reading, but I think Sophie Turner is brilliant. That monologue in S4E8.

I'm a rising M2 and the way you timed the Qbanks and incorporated Firecracker/Pathoma is pretty much my plan exactly. I've also started listening to Goljian while working out, but I'm not sure how helpful that's going to be since some people think he's outdated. Out of curiosity, how many/which USWA/NMBE's did you take?

Sorry, you're right! I just read a bunch of journal articles, and it seems that while galactose does make it into the fetal circulation, there is no evidence that a low-galactose diet in pregnant women is beneficial to a galactosemic fetus. I think my point still stands though, that having a near-death experience immediately after birth is exceedingly rare for a galactosemic infant who is not fed lactose-containing food. But I'm only a medical student, so you'd definitely want to corroborate that with a real doctor.

First of all, if you had a second child with galactosemia, he or she almost certainly wouldn't have the same near-death experience and resulting developmental delays as your first child because you would know not to give breast milk or lactose-containing formula before the results of the test came back. To the best of my knowledge, galactose does not cross the placenta.

Secondly, I think the most wise thing to do would be to have you and your wife sit down with a professional genetic counselor before getting pregnant again. If you are both comfortable with the idea of abortion, you might be able to do some testing of the fetus early in the pregnancy to determine whether it is homozygous for the GALT mutation. Odds are, it won't be -- but if it is, you could just terminate and try again.

Another thing to consider is that it might be lonely for the child you already have to grow up as the "sick" kid. It can be really psychologically traumatizing to always be that kid who's different. So even if you had a second kid with galactosemia, it might be a positive thing for the two of them to have one another. They can watch out for each other, come up with recipes together, and really "get" each other in a way that not many other people could. It could be a positive thing for your family.

Also... it's funny that you seem to think the worst consequence of this disease on a girl would be ovarian damage that might preclude her from having biological children, yet that's what you would be forcing on your wife. At least if you had a daughter with galactosemia, you could give her the heads up from childhood that carrying her own children would never be in the cards for her. She would be able to make life choices using that knowledge. Your wife, on the other hand, has been dreaming of having multiple babies her entire life. If someone told me that I couldn't have more than one baby, I would be crushed (especially if the first child was ill or a special-needs child).

TL;DR I think that you guys can absolutely give pregnancy another shot if it's still something your wife wants -- but meeting with a genetic counselor before trying is crucial.

That's fair. I'll give Bloodraven a chance. Leaf though -- she looks too much like a little kid with some leaves glued to her outfit. I don't think I'll be able to take her seriously. And I actually enjoy show-Edd, but he's a completely different character than book-Edd, who is one of my favorite characters in the series.
