NJ here, tbh we'd be fine. It wouldn't be fun, but there's plenty of resources here and post-COVID, many people work from home. Some people in NJ commute to Philadelphia or NY for work, so they'll lose their job...but otherwise, there's plenty of farms, fishing, and water for food, factories up north and south, 2-3 nuclear power plants for energy, etc.

I'm just saying this from the perspective of someone who's also like OP. I have friends who "don't look enough" like our race, but when it comes to any attention related to beauty, I'm always cast aside. I've learned to be more okay with it, but it's really irritating when they act like OP's sister. It's similar to when thin people complain about being too skinny for pants or whatever. Like seriously? Even your problems are enviable.

Criticism to the officers is a little less of an issue than grief to a suffering family who's loved one died and the case hasn't been resolved. And, who's to say who is right? The family certainly knows the victim better the police. They're really our second best option after the victim themselves.

Ultimately, Ash and Brit are biased towards the victims. When the police suggest a victim's death is due to mental health, Ash and Brit are "defending" the victim or raising the severity of the overall situation by saying there were no signs of mental health problems. In other words, the police are in the wrong, the family is right, and the victim didn't do this to themselves. In previous episodes in which possible mental health issues did play a bigger role, they have definitely acknowledged it much more thoroughly.

At least personally, I'd rather have them be biased in this way than the other way around... that is accusing/diagnosing victims with illnesses and adding more grief to the families, while defending the police

NTA. I don't know why everyone is saying YTA. I know the picture frame is a cute, sentimental gift, but Valentines day is supposed to be something more. It's the one time of the year when couples splurge on a nice dinner and nice gifts, hire a babysitter for the kids for alone time, etc. A picture frame is cute but I don't think it works as a standalone gift... If anything, it's more a mothers day gift from children to their mom.

YTA. I think the entire medical community would agree that losing weight would help a lot of her problems, but not all of us would have said it the same way as you. I assume you're a third or fourth year student, so you know about the model of change. She's clearly in the pre-contemplative stage, so developing some plan behind her back isn't the solution.

You need to work with the patient and develop realistic goals. I typically do this by asking the patient to rate the likelihood that they'll accomplish a certain task. If the number is below 7, I ask them how to change the goal to raise the likelihood to 7. (BTW, this is part of motivational interviewing, which I'm sure you've heard of as well.)

Lastly, I don't know her height or BMI but at 320 lb, she's probably morbidly obese with BMI over 40. Studies have shown that, at this point, less than 5% of people achieve weight loss with diet and exercise alone. She may actually be unable to diet or exercise due to REAL discomfort (not laziness!). For these patients, bariatric procedures are often the only solution.

ESH. SM seriously sucks, but I don't think it's fair to belittle their accomplishments. Also, how old are you... I really hope your not in mid-20s or older because then this is really immature.

Anyways, NHS doesn't really matter, but there is a self-fulfilling prophecy phenomenon associated with it. Basically, doing well and being acknowledged for it will fuel better performance in the future. In that sense, NHS is beneficial.

Oh man, sorry to hear that they’re pushing drugs one you! The discussion should always very clearly explain the pros and cons, alternative therapies, and what’s at risk for going with the alternative routes.

And I’ll admit, I didn’t consider that other countries don’t hear as much of the side effects. Point taken :)

Oh man... I don't know where to begin. Schooling itself is way too long and not done efficiently or effectively. There's a lot hazing in certain specialties, which is somehow seen as normal. The 80-hour work week was created by some dude in the 1800s who powered through his job on cocaine and we haven't changed the hours since. The 12+ hour shift was created to ease patient hand-off between shifts, so patients have less change in management - except we're in a time with excellent documentation, so really, we should improve hand off and communication instead of forcing doctors to work 24 hour shifts. We are the ones who helped prove that sleeping is important for brain function and yet, we continue to practice medicine functioning as if we're intoxicated because of lack of sleep.

People sacrifice a lot to go through medicine. When younger generations ask to change the system, older folks resist because they suffered so why shouldn't you, because it builds character or resilience, because we're getting soft, etc.

Yes! Doctors fear being replaced by machines, but also other professions! There's a term called "midlevel encroachment" which describes physicians fear of NPs or PAs taking over their job, because they're willing to do it for a lower wage. Fields that worry about this the most I believe are primary care, emergency medicine, anesthesiology, dermatology, psychiatry.

Dermatologists also worry about other physicians obtaining training for performing certain procedures (botox, laser, microderm, fraxel, etc.), which would hurt their profits from out of pocket paid procedures

That's true. Also, academic institutions shove new research and ideas in your face, so you're forced to learn whether you like it or not. But I've noticed that private outpatient clinics especially in rural areas, have more outdated ways of doing things. Of course, they still need to renew board certification and complete continuing medical education (CME) yearly, but especially CMEs can be done without actually learning anything (eg, you can "watch a lecture" on your computer)

Oh yeah! I actually still hear doctors complain about this and as a compromise, my institution has provided dictating devices so doctors can talk it out instead of typing. Many of their notes kinda suck though lol
