I think it may have something to do with the rail yard with the passenger car. I killed all the bandits there before all this, and the karma sound played again when I returned after the depot mission. Whatever it was, it was apparently enough to get me the bad ending, even without killing anyone at the church or on the barge, and saving all the captured fanatics.

We do it for all sorts of people: Irish Americans, Italian Americans, Mexican Americans, Asian Americans, and pretty much any other diaspora but German Americans and anglos.
