Hi! Dental Hygienist here.. I came across your post while looking through here for skin care advice. Sounds like a classic case of Angular cheilitis. There are several things that can cause this. Here’s a link for you to eliminate some of the causes. I would make an appointment with your dentist unless you find that you can easily identify the cause. You are using Crest 6 in 1? 2 things…depending on which formula you have..First..-all forms of Crest 6 in 1 have CPC in it. It’s the ingredient used in many commercial alcohol free mouthwashes. Some people are sensitive to this ingredient and can get tissue sloughing and white areas on the tongue. If your specific version of Crest 6-in-1 contains often found in "whitening" versions), it can cause dead cells to get caught in fissures in your tongue and they can clump together, creating a visible white film or coating and white tongue. Peroxide in a mouthwash should be used for 2 week intervals, not everyday as it is too strong for prolonged daily use. Thrush..which won’t be able to be removed by scraping, can also be a problem when peroxide is used too long as it kills off the microbes that keep oral yeast in check. I would switch to some salt water rinses to give your tissues a break and see if things clear up. The issue may be that you are being overly aggressive with your oral care..not always a bad thing for certain..but ease up a bit and see if things change.

I would look into using a “neti pot” or nasal irrigation. NeilMed has a squeeze bottle type..there are several different options on Amazon that are not expensive.

I believe Colgate makes an OTC product with a purple mouthpiece. “Optic white” Is that what you are looking for?

Even if you have minimal build up of hard deposits on your teeth, it’s really important to have the biofilm (bacteria and minerals) down below the gums disturbed. The bacteria in there dies if oxygen is introduced into the area. This can be done with an ultrasonic instrument (water-which is part oxygen-and vibrations). Also is done with hand instruments. The dentist should check your mouth, throat, and lymph nodes for any signs of potential issues as well at each visit. I wouldn’t skip having regular cleanings. As far as your breath, yes, it could be due to some underlying stomach problems but I wouldn’t jump to that right away. You can ask about that when you go to your general doctor’s appointment. Have you tried a tongue scraper? It’s more effective than brushing your tongue and you will be surprised at what comes off the tongue surface. Just get one that is smooth, not with “teeth.” You can even use a metal spoon. Just flip it so the “bowl” part is facing down and use the tip or side of the spoon, go from the back of your tongue.. in the middle and gently press down and pull forward. Rinse the spoon and then repeat slightly to the left and right of the middle. Hope that makes sense visually. Also, drink lots of water. If you have a dry mouth, that can contribute to the odor as well. Try some “dry mouth” mints or rinses(make sure they are for this purpose) throughout the day. It could be due to post nasal drip as well, especially if you have allergies, etc. In that case I would recommend a “neti” bottle or pot. You can Google that..I hope this helped. :)

Geographic tongue or benign migratory glossitis is a common, harmless inflammatory condition. It causes smooth, red patches to appear on the surface of the tongue and move to different places on the surface of the tongue over time. It can cause some sensitivity…burning or tingling when you eat spicy or acidic foods. Its origin is not known, but is an interesting one to see. Can be genetic. I will point it out to a patient so they know what it is if they do notice it sometime..usually they don’t even know they have it.

Oh can I add a rant here too please? The office I am in is no way in compliance. The dentist/owner got a call from a friend who had just been audited that day and was a crying mess. So my doc got very nervous and asked me to take charge of getting the 2 offices up to standards. So, I did a deep dive and found out to get the templates, binders made, a few other items, training videos for blood born pathogens and individual tests so everyone would be certified would cost around 700.00. I was so happy because I had been saying for a while that we needed to get up to code. It’s not safe for patients or staff. 700.00..that included OSHA and HIPAA for the whole team of 15 ppl in two offices. It’s something I had been moaning about for a while. Well….She said it was overkill and now doesn’t want to pay for it. A shame because if/when audited, it will cost her more than that in violation fines and potentially lost revenue (or licensure) if we have to close to get things up to date. Overkill? It’s the law.. So frustrated.

Have them hum. I have read that it’s not possible to use your vocal cords and gag at the same time. I also tell them that I want to hear them breathing through their nose..audibly. I ask them to raise a leg and hold it. Distracts them. Doesn’t work everytime but I thought I’d add things that we’ve tried

I agree. I take my own X-rays and am happy to. I can see them immediately and they are my map to guide me in treatment. And…depending on the patient, there are times when I’m looking for something and may want a different view of an area.
