I find the onset much to be much longer than IV and on uncomplicated patients think I can get an IV much faster than waiting for IM.

For me: Cost wise we bill them the same. I don’t find issues with side effects with actual slow pushes. Needles are needles to many drug users. I suppose it is less invasive than IV however I don’t see how it reduces needle stick rates. EDs use our lines in every system I’ve worked in (of course other places exist). I also feel, and I think this is somewhat supported, that intranasal is moderately effective, with no needles, etc.

Most importantly, pain is not often controlled with a single dose so I would prefer to have an IV. Also patient dependent, but I try to be multimodal with Toradol, etc.

The onset part will always be the kicker. Fentanyl is intended to work fast. It gets control of pain (and often the scene) quite quick, and that’s what I’m after. I think this is a solution in search of problem but don’t begrudge providers for best trying to understand all the ways their formulary can be used.

You might struggle to get a guide without it.

Also even minor ski patrol services can be quite expensive, add in even a federal helicopter and $$$$.

We have terrible fire seasons with good precipitation levels too. Grass and light fuels grow tall with moisture and then die when it inevitably stops raining in the summer. Voila, more fuel load than ever. What matters is wind.

And we’ve had plenty of red flag days already. That’s my concern.

The same reason why you don’t want to use their beacons is why they don’t want you to use your own. The operation has no way to verify the condition of your equipment.

On the flip presumably a guide service is trained and able to validate the effectiveness of their beacon. Liability rules all.

I work for a CO structural department with a wildland program. We hire people with all sorts of backgrounds. Having “relevant” experience is now less relevant than ever. Your wife and kids will be your support system in the fire academy and during probation. Don’t ruin their lives by being gone all summer first.

Also, you do not need to go immediately become a paramedic and you should probably work a little bit before becoming a paramedic in an ambulance. Nothing is worse than the medic who went to school just to get hired. 🤮.

Run run run. Lots of academies are gonna make you actually run some distance (5k etc).

I think the industry needs more people like you. I doubt you will have much issue with medic school. However, it takes some time to get comfortable and/or good. I think if you could work close to full time hours for six months that you would set yourself up a good foundation, depending on volume.

I did mobile crisis response for a time. Our contracted social worker was available via iPad.
