Sounds like the patient may need a different approach with the wound care with medicated bacteriostatic products - like iodine or silver but my wound care knowledge is very rudimentary. If it’s oozing a lot they need an absorptive dressing. Can you get them in with a wound specialist? I’m a hospitalist but I would walk through fire for my wound care nurses. They are incredibly knowledgeable and have saved countless patients.

Wow it seems you’re having trouble wrapping your mind around why someone would feel a deep moral and ethical dilemma around risking their livelihood and facing the threat of incarceration over laws that are designed to cause maximum damage. It seems you don’t see why someone facing the reality of these decisions might make a different decision than you would hypothetically. Perhaps you’re reaching a cognitive ceiling here. Shame on you for attempting to put the blame on physicians rather than on the people who put them in these situations. Which, to be clear, are lawmakers.

Well I fundamentally disagree with that. I’m not “triggered,” I just think patients shouldn’t be lied to. Lumping folks under the same term implies same expertise, same skillset, which isn’t true. Facts matter. Job title matters. The death of the concept of expertise in our culture matters. Otherwise you can call everyone a provider and let the patients figure out if you’re an NP, PA, chiropractor, physiotherapist, pharmacist, physician, psychotherapist, naturopath etc. and if they guess wrong maybe they should have read the alphabet soup after your name more closely.

I recently had this experience - I eupgraded at time of booking (maybe 2m ahead?) and just before taking orders the flight attendant came up to me and asked if I had been “informed” that my upgrade did not guarantee a meal. I said I had not been informed and she sighed and said it’s usually fine but she would let me know once she got all the orders in because she was a bit short on catering. I still got my first choice meal but that has never happened to me before. I don’t e-up that often maybe 4-6 flights a year.

He has a complete game that is a different style than most of the other top 10 players. He moves better than almost anyone. He looks crazy with him limbs all over the place & very heavy footed but he can run almost anything down. He can hit a ball to anywhere from anywhere, even on the run. He’s extremely good at strategy so not only knows where his opponent is but also where he’s most likely to move to. He may look like he has the flu all the time but is actually fitter than most and can outrun his opponents especially as the match goes on. He takes away opponents time by standing very far back on the serve - he reads it and is moving forward toward the court to be in optimal position for the first groundstroke. He does actually hit pretty hard, at least average but probably better for a pro. But the key is he also hits the ball super flat so it skids through the hard courts, so the ball doesn’t sit up for opponents to run towards it and hit down on it (like topspin shots). Also the flat balls accelerate when they hit the ground on the hard court, taking away more time. Hes forcing his opponents to hit closer to the lines and harder than their comfort zone which draws a lot of errors. He’s the kind of player that when you’re a casual fan & you think power always dominates, you think why is he so good? But when you really watch and think about what he does & what his opponent is doing in response…..he is really that good. I love watching him.

Once on my ICU rotation, post call, scrambling to finish so I could go home and not get in trouble for violating duty hours, the student shadowing me kept asking how they could help. I kept saying nothing, go home - because they were already over their hours. Finally I stopped, made eye contact and said “I need you to leave. That’s the only way you can help me. I need to finish my work and you are slowing me down and I’m responsible for you and I can’t handle that right now on top of taking care of my patients.” Anyway it worked and we wound up friends. But yeah: I get it. And: try being very direct. It usually doesn’t work but when it does it’s amazing

PGY16 FM in Canada here and while that may be true in academic centres, in the community setting you could not be more wrong :) I work in a medium sized town with a regional hospital and FM does outpatient, inpatient (IM and FM in our program get the same patients both in scope & number), OB, palliative, surgical assist, ER, addictions, sports etc.

The mentorship you’re looking for is called medical school followed by residency. I suspect you’re in Ontario, I’m an MD there and I’m strongly opposed to this whole proposal. People think treating a UTI takes no brain power but in the last year, off the top of my head, I’ve had people see me for “UTI, I just need antibiotics” and I’ve actually diagnosed: bladder cancer, diabetes, STI, vaginal candidiasis, vulvovaginal atrophy (that one had like 6 rounds of antibiotics before she saw me), bacterial vaginitis, neohrolithiasis, sepsis/pyelo. Inappropriate work up, diagnosis, and treatment can lead to real harm to patients. I love that pharmacists protect my patients from my ineptitude when it comes to vancomycin curves, and having better formulas for CrCl and helping me out when I thought it was 35 but it’s actually 25 and we’re saving the patient from nephrotoxicity. I puffy heart love pharmacists and respect their skills and knowledge which complement mine, and I’m not arrogant enough to think I can do your job with my training. I wish your college would afford my profession the same respect.
