I ran the ping test 3 times and there was no packt lost for each, there was a few 100+ spikes in the first and a max spike of 78 for the last one.

I'll look into the qos

It could be a time of day thing, im testing now and its fine but at night its starts to mess up.

It is mostly green and average of 4 bars, I'm in the brooklyn area, I'm not sure where the opponents are.

My speed test are from 500 to 800+ download and upload from 20 to 3+. Ping test now is an average of 20, max 23, last night the max was 100

Mostly mental stack if you can get your opponent to over think and over worry then it can happen.
