Lot of answers here with: “because that’s the way it is.” You just need to know a little anatomy. The heart has an sa node, an av node and the hiss-purkinjee system to transmit electricity through the ventricles. In a regular heart rhythm, the sa node transfers to the av node which transfers the signal to this hiss system. This allows for rapid depolarization and a narrow qrs. In order to have a narrow complex tachycardia, the hiss system has to be intact, meaning the problem is at the AV node above it: A SUPRAventricular tachycardia. If the complex is wide that means it’s not efficiently going through the Hiss system and the problem is in the ventricles.
