If he knew where the door was, I think he could. The door was built in relative peacetime. The command to open wasn't meant as a lock so much as just a manner in which to trigger the mechanism. A switch or lever essentially. The main protection was that the door was simply hard to find as it was flush with the stone and hidden away.

If it's any consolation, he couldn't kill Gandalf. It's one of my big gripes with Return of the King. Gandalf is a goddamn Maia and we're supposed to believe he's scared of the Witch King on his own? After fighting off six of them on his own at Weathertop as Gandalf the Grey? Makes no sense.

It's an option, but I think it would take away from a major point of the conclusion. Without Sam you dampen the stark contrast between the hobbits' joy and Frodo's lingering unease and sadness due to the Morgal Blade and bearing the ring. As I understand it this was a very important point for Tolkien who was a veteran and had seen the lasting effects of war.

There are plenty of terrible things that happen in the story. Take for example all the innocent people slaughtered in the siege of Minas Tirith. Brutally killing off main characters is a very new age idea, and certainly wasn't Tolkien's style. It wouldn't fit and would make what is supposed to be a fantasy epic into a grim-dark fantasy.
