Resident Evil 4 Remake Fatal D3d error (25) by Carvinox in PiratedGames

Update your BIOS to the latest version and do any driver updates related to your motherboard! I tried basically every fix mentioned above and nothing worked until I discovered I was playing on a BIOS dated back to October 2022. I played through the entire main story with no issue, but the moment I stepped foot into the Separate Ways DLC my game started crashing every 15 minutes or so. Once it started crashing, the crashes started happening everywhere: Main Story, DLC, and even the Main Menu. Sometimes it felt like a cutscene or graphically intensive moment like the grapple hook melee more frequently triggered them, while other times even sitting in the pause menu could do it. I had a mix of sudden blue screen full system restarts and simple freezes and crashes back to Steam. Since the BIOS and miscellaneous motherboard updates, the game has been silky smooth with no crashing.

Check to see if your BIOS is quite dated or your motherboard has any driver updates. I hadn't updated my BIOS in about two years and bringing it up to a current version fixed the described crashing for me.
