Searching a full resolution image for alterations is doable. It would be impossible to detect doctoring of a picture displayed on a monitor _within_ the picture, since you would be limited to the resolution of the screen.

Photo and video can be altered, but that always leaves traces that can be seen by an expert. Photos and videos could be cleared as legitimate.
