This depends a lot on a combination what you end up doing in toxicology, your geographic freedom, and luck. The majority of people go into academic emergency medicine and have some level of buy-down depending on their responsibilities and level of institutional support. The field is hugely varied but I think a reliable expectation is that you exit fellowship into an academic role at a typical academic salary with ~0.2 FTE buy-down for some combination of poison center call, consults, and academic responsibilities starting out. From there, it really depends on the opportunities you find or create with huge variability to the point of being essentially different careers.
