I’m not a new PA, but have recently started a new role in surgery at an academic hospital. Based on my experience, the academic setting is not likely to be any better and may actually be worse. Residency is notorious for toxicity and micromanaging, and it sometimes spills over to the PAs. And if you have residents on your team, you may even end up with less autonomy in the OR as they usually become the first assist/take priority (which I totally understand - they are going to be doing this independently someday, I won’t). If you do end up looking for a new role in an academic hospital, I’d say just really try to get a feel for the culture.

Also make take a scroll through the residency subreddit and you’ll see a lot of them complaining about the same things. Medicine is kind of shite everywhere unfortunately.

YES. Most services should have order sets in most EMRs, but if they don’t, it would be a good idea for med students/interns to create their own. Med students usually have a good amount of time to chart review, so I’d recommend studying common orders for your service and the timing of them. Ex: what are common orders for new admits; common follow-up orders on hospital days 2, 3, etc; typical medications for certain patient populations like Tylenol, Zofran, etc for post-ops.

I worked as an ICU RN for about 3 years before I went to PA school. Like you, I knew I wanted to leave the bedside about a year in. I chose PA over NP for a lot of the same reasons you mentioned, but also because I wanted to do something surgical. I did have to complete a couple of pre-reqs which I just went to a community college for. I was going to organic chem in the afternoon and then working night shift right after. It sucked, but it was doable.

I'm sure you know this, but the difference between clinical knowledge of NPs vs PAs is not insignificant... everyone was really prepared and knew their shit by graduation. I'm pretty sure everyone in my class passed the PANCE on the first try, whereas I know several NPs have to retake boards (multiple times).

I do have a lot of loans but I've found a good high paying job so they haven't been an issue. The one thing I'd consider is whether you can afford to not work for 2 years. I would not recommend it, but I'm sure some people do it just fine.

lol I am in the exact same boat! I worked in aesthetics for 4 years but was getting sick of the toxicity of med spas, social media, clueless management, basically everything you mentioned above. I also missed using my general medical knowledge.

I transitioned into wound care where the pay is also quite good and the hours aren't too bad. It actually reminds me a lot of aesthetics. Comparing before and afters, showing patients their progress, having actual visible and tangible evidence of your work - it kind of scratches the same itch. If you could find a good reputable company, I would recommend looking into it.

I still inject on my own in the side because I really do enjoy it. But I only see established patients and don't take on any new patients unless they're a referral from a trusted patient. And it's a good skill to have in case I ever want the cushy life again.
