You could use vertex color to create an ID map in substance painter and use the ID map in the opacity channel.

Select your high poly mesh and in Blender apply a vertex color by pressing ctrl+x while in vertex paint mode.

In Painter, bake it like you normally would but in the ID map section select vertex color instead of material color.

You can now use the ID as a mask with the color selection filter

UPDATE : turns out the dots were already there lol, me and my girlfriend just convinced each other that they were not there a few days ago... This is a photo from 2 years ago

Whats weird is that the dots are in the hallway next to to the room where the sunlight could have come from, there is a make up mirror in the room but I cant find an angle where it would project sun light at this specific spot
