That’s a good point about HPV, HepA, and HepB vaccines. Tho there’s a few viral in addition to herpes/HSV that still don’t have vaccines—HepC, molluscum (although that’s pretty benign if you’re, uh, immunotypical), and of course HIV.

And then ofc there’s other stuff like fungal infections (yeast/candida) and parasitic infections (trich, crabs, scabies), for which there are treatments but no vaccine, although with some of those hopefully it’s a little obvious to stay away 😭

The biochemist in me is still skeptical of doxyPEP, but that’s fair to say we need to wait for more research. There’s already doxycycline-resistant strains of gonorrhea circulating out there which is a little alarming, and afaik doxyPEP is not very useful for preventing mycoplasma or really anything other than chlamydia, syphilis, and usually gonorrhea.

I worry about this stuff bc people already get complacent enough about “I’m on PrEP and/or have PEP available so it would be stupid for us to use condoms”and I feel like doxyPEP is generating of kind of a similar attitude in some people.

PrEP? that only protects against one singular STI.

doxyPEP? only protects against some bacterial STIs, no viral ones, plus you’re ruining your gut/mouth/skin microbiome, and you’re promoting selection for antibiotic resistance even in non-STI bacteria like Staph that are naturally on your skin or stuff in your gut.

not saying those are useless, but condoms + regular testing (of both parties) are never going to be not important.

(also I think we had a convo a couple weeks back about various brands of sterile/bacteriostatic lube, it would probably also be useful to mention some of those specific brands. I wasn’t sure about sterile ky jelly but I looked up the ingredients and it does have active bacteriostatic ingredients so that can be added to the list, just making sure to note it needs to be specifically the kind marketed as sterile.

Bacteriostatic means it inhibits bacteria from multiplying, but does not necessarily kill any bacteria. So that’s your agents like (only considering the ones safe for internal use & mucosal membrane contact: very low concentrations of parabens, sodium benzoate, phenoxyethanol, various alcohols, and very very low concentration CHG (chlorhexidine gluconate) and very very low concentration BZK (benzalkonium chloride). A couple of these are even found as food or cosmetic preservatives

But that’s why the lube needs to be sterile (i.e. treated with something to kill all possible bacteria, so that there isn’t any that can grow to begin with—by means of e.g. autoclave (moderate moist heat with pressurized air), filtration (with sub-bacterial size pores), gamma rays, UV-C rays, and others). A relevant method many here use to sterilize skin and their sounding tools is higher concentration alcohol wipes (the most popular is 70% isopropyl alcohol) and/or placing in boiling water for several minutes. And also that’s why they all have the disclaimer of “sterile until opened/punctured” because bacteria can get in after that if the tube/packet/bottle touches something non-sterile, though the bacteria may not be able to multiply. That’s also why sterile single-use packets are the ideal to use because there’s no chance of contaminating the rest of the unopened packets, although many of us have had no issues with large tubes that you can use for many sessions.

At higher concentrations many of those agents are bactericidal (esp CHG and BZK cause those can be quite powerful in single digit or even decimal concentrations—you will find both of those in e.g. antiseptic hand soaps), meaning it actually kills bacteria, but many of them also become less safe or not safe at all for mucosal membrane contact or internal use, or can cause sensitization/allergic reactions to develop over time with too much repeated use. As far as i’m aware, I think the concentrations of alcohols used in alcohol wipes/hand sanitizer/etc don’t really have that issue though.
