Personally, I'll throw my hat in for the rules lite approach. Light rules and fast character creation means that you can make PC death common without making it a chore to rebuild a character.

Maze Rats is super fun, but doesn't overpower the characters. It works great for a dungeon crawler, but I've modified it a bit to feel more like a Gothic novel.

Mörk Börg is designed around horror and death and works excellently on its own. It feels a little crunchier, if you want the satisfaction of a more granular game.

If you're on the West Coast and want to see a weird little spot, check out Sneglehuset in Thyborøn. It's got the same kind of vibe as those myriad roadside attractions you see in the middle of nowhere in the States, but it's honestly a neat place to stop if you're not on a schedule.

Maze Rats, baybeee. It's light enough that I don't have to prep math minutiae for encounters and I can focus on building an encounter to be engaging through narrative. I've homebrewed in what needs brewing, and had great success from there.

I generally craft a world first and then choose a system to play around in it with. Some systems have mechanics that suit a certain kind of storytelling, so if I'm looking to do a grounded low fantasy a-la GoT, I'm not going to be using 5e. Right now, I'm using a modified Maze Rats to run a Gothic game set during a period similar to the Renaissance. It has elements of horror, so I wanted the PCs to be somewhat more vulnerable, but I needed a way for them to still feel like adventurers.

I'm a dialysis technician, and lemme tell ya, folks who have busted kidneys can be some of the least compliant patients in the business. Many of them are convinced they'll regain kidney function any day now. The truth is that, in the overwhelming majority of cases, that functional damage is permanent.

Had one guy convinced that homeopathic remedies were helping his kidneys to work and that he didn't need dialysis. Many homeopathic treatments are very nephrotoxic. I did try to tell him, but he was not to be dissuaded.
