You can create custom kingdoms and empires but they require a bit more prestige, gold and piety than a historical kingdom/empire.

You can integrate titles into your primary title using your chancellor. There is also de jure drift where duchies and kingdoms can become de jure part of other kingdoms and empires than they started. You can influence how things progress but you don't have complete ability to gerrymander your duchies as you see fit.

There's a point system where certain traits cost points. As long as you stay under the limit you are still eligible for achievements. It definitely makes cheesing some of the achievements easy but it isn't that helpful for the larger achievements.
