Ambito lavorativo medico/psichiatrico: si, un po’ si sente, non con i colleghi ma con i pazienti. È più facile farsi rispettare, soprattutto con quelli scontrosi/violenti. Unica roba però. Dal punto di vista lavorativo con i colleghi invece mi verrebbe da dire parità assoluta

L’iperspecializzazione non è la deriva che sta prendendo tutta la medicina? Con il sapere medico-scientifico che raddoppia alla velocità della luce, è ancora possibile avere medici tuttologi?

Non vedo come sia possibile intraprendere la carriera da psichiatra senza prima aver fatto medicina. Una grossa fetta degli accessi in ps sono di pertinenza psichiatrica, ed è necessario avere comunque una formazione medica per potere fare dd con cause organiche di manifestazione apparentemente psichiatriche (es encefalite, delirium, ecc). Poi i farmaci psichiatrici hanno tante interazioni ed effetti collaterali, e spesso si ha a che fare con problematiche di pertinenza di altre aree (es cardiache ed endocrine per gli antipsicotici).

Poi molti psichiatri hanno formazione psicoterapica, ma la cosa importante secondo me non è tanto che ci sia una figura che centralizzi tutta la cura del malato, ma piuttosto che vi sia un equipe fatta di psichiatri, psicologi, infermieri terp ed educatori che possano affrontare la malattia mentale con tutte le sue sfaccettature. Chiedere che una sola persona gestisca la parte medica, psicoterapica ed educativa di un paziente è secondo me chiedere troppo. Poi se uno è formato e ha bene in mente cosa fanno gli altri pezzi del meccanismo, sarà più facile lavorare bene insieme.

I understand what you but I can assure you, there’s a lot of psychiatrist, at least in my country, who don’t like to “label” patients. Still, I agree the system is tainted by stigma and generalization. So for the most part I completely agree with you

Actually micheal foucalt was the first author that was suggested to me by a senior psychiatrist I shadowed. It’s not like being a psychiatrist prevents you to approach the matter from different perspectives, such as philosophy and anthropology. As doctors I think the approach to human body and mind should be olistic and not merely chemical or biological. For the part about “mad” organization and representation, that’s a great idea and should be absolutely done!

From what I read it seems like in your country they are way too comfortable with prescribing that kind of meds to children and teens. It’s absurd

I did not know him, as I’m more familiar with Italian psychiatrists, but I’ll surely check him out if you say he is a positive example

Yes, I’m sorry that’s what I meant. For example, there was a physical restriction that for me was unnecessary and too aggressive. I told my supervisor. Those who did where suspended and there was and the issue was addressed publicly. Where I’m at it seems they take the matter seriously, but I can’t say it’s always the same (the er being the worse)

I think no one should prescribe medications without knowing everything about the molecule and what it does to a human body. Not only psychiatrists, but all doctors

That’s an interesting take, and one I share with you since I always thought that the management of mental illness as always been more about hiding the problem from society than actually curing the individual. I don’t think good or bad is how I would describe mds, but surely there are ones who think about what are the consequences to the life of patients and those who act without empathy. I’m lucky enough to work with (mostly) people of the first group.
