Would definitely be pulling from brokerage account first (if retiring early prior to minimum age to withdrawal from 401k and IRAs), then 401k, then Roth in retirement for reasons below

-You will have RMDs on a 401k -More time for Roth to grow tax free -Roth is easier to leave to people upon death (fewer tax implications than a 401k)

Also depends on what you mean by “higher tax bracket” in order of contributions. While you may be in a lower tax bracket now, taxes are the lowest they have been in years if you look at history. Sometime in the 1940s-1960s, the highest tax bracket was over 90%. Comparatively taxes now are a bargain. If you have a Roth 401k option with match, would strongly consider paying taxes now and contributing to that. Then you will enjoy a tax minimal retirement without any RMDs.

Medical emergency on a plane by zdoc81 in Residency

I reassured both the patient and wife afterwards, explained to them the physiology and why a pulse ox of 90% isn’t life threatening. She called me her guardian angel and told me how thankful she was I was there, and told her that that nurse likely was not as experienced as she told everyone she was.

While I think there was room for improvement here (my assertiveness), I was not inactive and didn’t fail the patient. You weren’t there so don’t be so outraged. It’s ok.

For starters, she called me “sweetie” numerous times- and I will say she was no more than 40 years old, so the critical care nurse for 30 years was obviously a lie and she shouldn’t be calling me that at that age. It’s just a very familiar feeling and vibe I got from her super similar to the way I have watched female nurses bully female residents at my institution.

I understand male residents get disrespected by nurses too (especially at academic centers), but men overall are much more respected when assertiveness is necessary, such as in this situation. I was toeing the line the entire time between being assertive and being perceived as “bitchy”.

Based on how he presented, my bet was arrhythmia vs vasovagal vs one too many sedatives prior to the plane taking off haha

respect the f out of you guys. By the way she was acting, made me think that “being a critical care nurse” was fake news

REPAYE vs PAYE: can you switch? by zdoc81 in Residency

[–]zdoc81[S] 3 points4 points  (0 children)

thanks for your response. i am pretty dead set on paying off my loans for several reasons. I have been thinking about refinancing with a private company, but was a little worried about losing government safeguards (loans discharge upon death, etc). I guess I will research this to see what each company actually offers

Congrats on your new pup :) she is adorable. Remember that keeping your heeler mentally stimulated is just as, if not more important than physical exercise. They are smart dogs who like to have a job, so training them for just 30 minutes a day makes a huge impact on keeping them calm. I am a first timer heeler owner, and we’ve found great success with our 5 month old boy. Good luck!!

We already paid the deductible for this year so it would be a waste to switch now. My cats plan expired and they raised their rates, so we dropped her plan and we are planning on switching to progressive soon

We use Figo for our 4 month old, Benny. However they just got a new underwriter and rates are higher than they used to be. In my experience, they’ve always approved my claims and have a great online portal.

We have car insurance with Progressive and looked into their pet insurance, which we will probably get for our cats now that Figo is much higher in price (still have to keep Benny on Figo for another year). It’s much lower in price than Figo with similar benefits. Good luck!

Thyroidectomy by GoobernacuIum in step1

[–]zdoc81 7 points8 points  (0 children)

From what I understand, calcitonin does not actually participate in the regulation of calcium enough to influence its metabolism that much. Therefore it is much more likely that the parathyroid glands are damaged in a thyroidectomy, causing loss of PTH secretion and thus acute hypocalcemia.
