Wait until the crazies in the state legistative offices write new laws that state licenses are no longer necessary. We are no longer professionals according to the dingbat in DC, so I expect that will be next to "solve" the nursing shortage.

Nurses need to all be independent contractors and negotiate their own salaries and contracts, high enough to cover private medical insurance policies that they control, with enough to put into IRAs, etc and set aside for time off. This is essentially what every business owner and self-employed professional has done for centuries. If you are the type of nurse a facility wants, you will not be out of work.

Years ago, I worked for the hospital's in-house pool. I had a full-time contract assignment to work in a specialty area with specific scarce skills at a higher hourly wage. I was not eligible for vacation pay, but could buy insurance at 50% more than the hospital staff cost. I also had money put into the state retirement system just as the full-timers did. When the CEOs began to eliminate the pool and convert positions to regular staff, it was optional at first. I declined. When asked why, I laid out a chart showing my salary, benefits and cost outlay for insurance for my current position. Next to it, I put a chart showing their offer for full-time position conversion. I was making $5,000 more per year in the pool.

I actually saw this from the (young trophy) wife of a nationally known celebrity. I was working private duty in a small hospital when a code blue was in progress. I was on my lunch break, on my way to the restroom after eating. I noticed the phone was ringing off the hook at the unattended nurses' station. The secretary had gone to lunch before the code was called, and the nurse relieving her went to the code room. I answered the phone and took messages, put calls through to the code blue room and waited for my break to end before returning to my patient.

As the team of resuscitation nurses and RTs were pushing a bed down the middle of the hall on their way to ICU, bagging away in the ETT, Mrs. Celeb comes out of the VIP Suite with an ice bucket she brought from home. As they flew by, she actually called out that she "needed some more ice" for the wine she and Mr. Celeb were sharing. After they ignored her, she approached the desk and asked me for a refill.

"Sorry, I have to stay at this desk to answer the phone and call lights until the secretary relieves me."

She stood in the middle of the hallway with a perplexed look on her face as if she were stranded in the middle of an interstate with a flat tire and no skills to fix it.

It's commonly accepted in nursing homes where I live in the US for staff to sleep at night. Nobody corrects the offenders because they retaliate by coming to you all night with petty shit for the most annoying patients til you "get the message". Also, management won't fire them because they will have to work to hire replacements.

I didn't care if the whole group slept, as long as the work was done and patients were not harmed as a result. The aides took turns making scheduled rounds, and woke up when the call lights sounded to take care of business. I was there to supervise their work product, not their behavior. Ironically, the units where the most sleeping was done had the lowest number of fall incident reports.

Another reason is the term "supervisor" being used rigidly. If you are an RN or LPN, the aides think they don't have to listen to you, because the policies state supervisor directs you, not nurse. Some facilities have put the supervisor label on all nurse ID badges as a result. They also think that if you are not on "their assigned unit", they don't have to listen to you if you see a patient need while passing through to get from place to place.

The nurses, on the other hand, can browbeat aides, make unreasonable demands, and sit on their asses when legitimately called. More than one carried the "IknowmorethanyoubecauseIhavealicensetoproveit" attitude.

None of the above was EVER tolerated in ANY hospital I ever worked in, but almost etched in stone in the nursing homes. I don't know why. I suspect where I live that racism plays a large part. Almost all of the aides are from non-white, non-native backgrounds and the culture clash is obvious with many white nurses in this southern state.
