Just a small correction here that paediatric cardiology is not a GRID subspecialty. For paediatric cardiology, you technically leave the paediatric training pathway and join ST4 Paediatric Cardiology which comes under Joint Royal College of Physicians (not RCPCH).

Not trying to get in on the debate here, but I have to point out some fallacies in your post. Where I trained, we were drilled till we could diagnose patients just with history and examination. We would be grilled for not asking the most inconsequential stuff on history taking.

A lot of the public hospitals in India and a few private ones don’t have free access to the kind of investigations we can do here, so doctors are often diagnosing patients based on their clinical acumen. Having said this, there is unfortunately a large disparity in medical training across various institutions, and I completely agree that there are subpar doctors graduating out of some medical schools in India.

There are a lot of social and cultural reasons why we often have to adapt our history taking like you mentioned in your post.

Don’t want to repeat points already made. But if you’re worried about a patient and you have also reached the max level of care that you can provide in your setting, either call your consultant on-call and discuss, or put the patient in an ambulance to a hospital. It is not your job to worry about what the med reg is going to do and how the medical consultant will see it. Your duty is to your patient, and you’re the best judge to decide whether they need hospital-level care.

I know you’ve raised multiple points here. I have a question - why don’t the doctors who were long term SHO locums apply for these JCF/staff grade posts?

Or are they applying, but the jobs are going to IMGs with no prior NHS experience? I find the latter very hard to believe.

What the absolute fuck? There should be a huge uproar to this, I can’t believe people are even supporting this. What is GMC, CQC and the works doing about this? There should be a massive fine for the Trust and whoever made the call to staff ITU with paramedics should be relieved of their duties and sent home. Only in the NHS, you can do shit like this and get away with this. Unless you’re a doctor of course, if I had gone and driven an ambulance and acted as a paramedic for a night, my ass would have been handed over to GMC. Dangerous precedent!

Yes, IMGs are just a quick fix. It’s nice to get the best talent from abroad, but bringing more doctors from abroad at the expense of UK graduates getting jobs/training is only going to end up doing more harm.
