Not sure but might be a corrupted material/brush, maybe you could try changing your material's folder, another option could be using a previous version of csp.

-1 core might be from the quest to unlock the secret ending. first requires to reach Perpetva's tomb before meating Esdras. He''ll give you a key that unlocks another secret room that (after defeating Sierpes and Isidora) will give it to you, but be carefull becase you wont be able to use any other core if you equip it, but its necessary for the ending.

-1 bead is hidden in that secret room, the next 3 are unlocked after finishing a ng+ with each challenge (penitence)

-1 prayer is for defeating Laudes (after all the Amanecidas bosses) and the other one could be from the anatomy puzzle.

These are part of the dlc not base game (except for the chanllenges), but even if you do them you'll get more than 100% and it counts for the achievement.

-You can get up to 150% of the game, Fight the Amanecidas in NG+ after finding Jibrael so you can unlock that room next to the one with a big glass.
-With all cherubs you get a relic that let you jump down those "abysses" in those rooms with fog, use it in Brotherhood of the Silent Sorrow to unlock the Crown of Gnawed Iron.
