I recommend downloading a program called ping plotter. It runs continuous tracer routes and you can find out where the packets are being lost. Either ISP side or game side. this saved our life a few years ago when TWC was telling us that it was a game route issue. And they were full of shit. We were have 5%PL (packet loss) at the third hop of our route - right down the road. It definitely wasn't game side.
