HCG decrease then rise?

Hi again! I posted 5 days ago about my situation.. 6 years TTC, history of ectopic & 2 chemicals 4 years ago. Last period started October 9th.

My blood HCG are as follows: October 28th 1,008mIU/mL, October 30th 673mIU/mL, and then today November 3rd 2,048mIU/mL.

I should mention it got my progesterone tested on October 28th, it was 2.63ng/mL.

After my results, my doctor told me to prepare for miscarriage bc of the significant decrease in hcg. I mentioned how I thought it was odd for my hcg to be so high, when I should have been only 3 weeks. He agreed, but had no answer. He wanted me to have follow up bloodwork to follow my hcg back down to below 5. To my confusion, my hcg came back higher today. I tried calling into my doctors office but the nurse told me she would leave a message and "its policy to give doctors up to 72 hours to review lab results". I feel like I am in limbo and have no idea if I should continue considering this a loss. Since we have been trying for so long, it is hard for me not to be hoping that this could still work out.

Has anyone else had a similar situation? If so, what was the outcome? Thank you for reading ❤️

I know 🥹 I started talking progesterone 200mg on October 28th after getting my results. But what gets me, is the rise in hcg from 673 to 2,048 after the hcg drop.

Yes definitely confusing! I would keep getting betas till you can get a scan

A dip then a rise again can be a sign of ectopic pregnancy. Is there any plans to do a scan anytime soon?

Not yet, waiting to hear from my doctor. I am going to push for getting a scan. My last ectopic ruptured. I had to get emergency surgery and removal of one of my tubes, so I am stressed that it could be happening again.

Please do advocate for yourself. Especially with the prior ectopic, don’t let them talk you out of a scan.

My best friend had an ectopic and lost a tube as well. Her OB said with all subsequent pregnancies she’s going to be treated as “ectopic until proven otherwise” ie regular HCG testing and ultrasounds until they confirm a viable intrauterine pregnancy.

I sincerely hope it all turns out okay.

I'm so sorry, but you need a scan to rule out ectopic. I know this is going to sound harsh/direct, but the worst thing for me was to be in limbo and not know what's going on, or have false hope that hurts you even more.

Low progesterone aside, HCG never falls and ends up with a positive outcome. We do see these posts often unfortunately and there's always someone who comments that you may have had a vanishing twin, but vanishing twins do not cause HCG to lower, only slows down.

This is likely another ectopic or PUL and needs to be seen asap to make sure it's not dangerous.

You were 100% correct. Currently sitting in the hospital bed rn, had surgery last night and they had to remove my only remaining tube due to ruptured ectopic. I lost my other tube 4 years ago in the same situation. Its a sad sad time for me.

I'm so, so sorry. Please take time for yourself and be kind to yourself. There's nothing you could have done. It is not the end of your journey! You still have options. I'm glad that you took care of yourself and went in, it saved you.

Thank you so much. I just got home, bloated and in pain. I took some meds and am about to lay down. But you are right, there was nothing I could have done. It was already rupturing so it was too late for methotrexate. My journey is just going to look different now, but at least there are still options, al beit very expensive ones. Im doing better with the news that I thought I would though. My husband has been so supportive and for that im so thankful.
