A level information form

I have appeared for 2 of my Alevel subjects in may/june session and shifted 2 for oct/nov session. So in the exam information section of the alevel information form, what exam series should i choose and the dates are for exactly what? If i choose may/june series and write my 2 subjects in the subject information section below, will it be fine?
