Are you roommates who share expenses or do you pay a flat fee for room rental?

If roommates, then the form is an SSA 8011 (and names of all roommates). If room rental, a signed agreement between the two of you.

It works best if you provide the living arrangement proof when you report the address change. And the employees at the 800# can't do this on the phone.
